先天性心脏病封堵术后经胸超声心动图血流动力学随访观察 被引量:2

摘要 目的评价先天性心脏病封堵术前后各房室形态及左室功能的改变以及超声心动图检测价值。方法应用超声心动图对32例采用国产封堵器行ASD、VSD、PDA封堵术患者术前及术后24h、1个月,分别对三尖瓣口、肺动脉瓣口、二尖瓣口血流峰值流速以及各心室容积、左室功能变化进行检测,用定量的方法说明封堵术对心功能的改善情况。结果先天性心脏病封堵术后及随访期右心室内径缩小,三尖瓣口、肺动脉瓣口血流峰值降低,右心前负荷的降低,短期内右心-肺循环高动力状态逆转,在新的前负荷水平达到新平衡;左室前负荷、收缩功能改善。结论应用先天性心脏病介入治疗根治了患者先天性的解剖畸形,纠正了血流动力学紊乱,患者左右心室形态、血流动力学进行性改善。 Objective To evaluate the hemodynamics changes of cardiac atrium and ventricle after transcatbeter closure of congenital heart disease and the value of the echocardiographic examination. Methods Thirty two patients with congenital heart disease underwent transcatheter closure by using the domestic made occluder device. Echocardiographic examinations were made to detect ventricuJar function before, 24 huors and 1 months after the closure.Results Right ventricular internal dimension was diminished, while blood peak flow value of tricuspid valve and pulminary valve were degraded and preload of right ventricular was degraded after transcatbeter closure and the following up period. Hyperkinetic state of right ventricular-pulmonary circulation was reversed, achieving the new balance on the new perload level. Left ventricular preload and systolic function were improved after the closure. Conclusion Interventional therapy of congenital heart disease can correct congenital anatomic abnormity, which rectificated disorder of hemodynamics. Cardiacmorphology and left ventricular function improve progressly after transcatheter closure.
