摘要
目的探讨经皮椎体成形术治疗骨质疏松性椎体压缩骨折的临床效果。方法采用经皮椎体成形术治疗疼痛性骨质疏松性椎体压缩骨折67例共72个椎体,术前VAS评分平均7.5分。其中54例共59个椎体直接在“C”型臂光机引导下,经皮经椎弓根穿刺,向椎体注射低粘度显影的骨水泥;13例术前先行闭合复位,再行经皮椎体成形术。观察术后症状改善、椎体高度及并发症。结果65例患者手术顺利完成,2例椎体穿刺不成功。62例术后72h内腰背疼痛缓解或显著缓解,VAS评分平均1.8分,与术前比有显著性差异(P<0.001),3例无效。59例分别于术后3个月和6个月进行随访,VAS评分平均为2.1分、2.25分,与术后72h内相比无显著性差异(P>0.05)。椎体高度:13例恢复或基本恢复,2例椎体高度丢失。骨水泥渗漏9例,胸腰段后凸畸形疼痛4例。结论经皮椎体成形术治疗骨质疏松性椎体压缩骨折,能迅速缓解疼痛,部分适合病例术前采用闭合复位,可恢复椎体高度,改善后凸畸形。
Objective To study the clinical effect of vertebroplasty on vertebral compression fracture with osteoporosis. Methods 67 eases, totally 72 vertebral bodies, of painful vertebral compression fractures with osteoporosis were treated by vertebroplasty since January,2003. The average VAS score of these eases were 7.5 points. 54 eases, totally 59 vertebral bodies ,were carried out vertebroplasty guided by C-shape X-ray machine; 13 eases were close relocated before vertebroplasty.Results 65 eases were successful, 2 eases unsuccessful in puncture. Backache improved or obviously improved in 62 eases within 72 hours after operation, average VAS score was 1.8 points. There was a significant statistic difference before and after operation （P 〈 0.001）. 59 cases were followed up for 3 month and 6 month , average VAS score was 2.1 points and 2.25 points. There were no significant statistic difference in the VAS score before and 72 hours after operation （P〉0.05）. 3 cases had no change in backache. The height of vertebral body： 13 cases restored or almost restored, 2 cases lost. Bone cement leaked in 9 cases. 4 cases appeared pain beside vertebral.Conclusion By vertebroplasty vertebral compression fracture with osteoporosis could acquire pain release, some cases could restore height of vertebral boody and improve abnormality by means of relocation before operation.
