摘要
目的:探讨黄芪注射液(AI)对急性脑梗死(ACI)患者的临床疗效、外周血NK细胞和T淋巴细胞亚群变化的影响。方法:61例ACI患者随机分为两组,治疗组31例用AI加盐酸川芎嗪注射液(LH)治疗,对照组30例用LH治疗。用先进的流式细胞分析仪(FCM)直接免疫荧光染色法检测两组患者治疗前后外周血NK细胞和T淋巴细胞亚群,并与31例健康人组作对照。结果:ACI治疗前CD3^+、CD4^+、CD4^+/CD8%_水平均明显低于健康人组(P〈0.01),CD8^+、CD16+56则显著高于健康人组(P〈0.05),治疗组治疗后CD3^+、CD4^+、CD4^+/CD8^+水平较对照组增高显著(P〈0.05~0.01),CD8^+水平较对照组明显减低(P〈0.05),临床基本治愈率与显效率亦明显高于对照组(P〈0.05)。结论:ACI发病后存在严重的NK细胞和T淋巴细胞免疫功能紊乱,AI治疗ACI的作用机制可能与其调节细胞免疫功能的平衡失调、减轻中枢神经系统免疫应答所致炎症损伤等有关,此为AI的临床应用赋予新的内涵。
Objective :To observe the therapeutic effect of Astragalus Injection(AI) and the effect of it on the activated natural kill cell (CD16+56) and T lymphocyte subsets (including CD3^+ ,CD4^+ ,CD8^+ ,CD4^+/CD8^+ ) in peripheral blood in the patients with acute cerebral infarction(ACI). Methods :61 patients with ACI were divided into two groups, which consisted of 31 cases as the treated group which treated by AI combined with Ligustrazine Hydrochloride and Sodium Chloride Injection( LH), 30 cases of the control group were treated by LH,and other 31 health adults as normal group. The lymphocytes in the peripheral blood of patients with ACI in different periods before and after treatment were immunologically labelled by dual color fluorescent monoclonal antibodies natural kill cell and T lymphocyte subsets with flow cytometry ( FCM ). Results :The level of CD3^+ ,CD4^+ and CD4^+/CD8^+ decreased markedly while the level of CD8^+ and CD16+56 increased relative during acute period of ACI as compared with health group before treatment; the level of CD3^+ ,CD4^+ ,CD4^+/CD8^+ in the ACI patients of AI group increased more significandy than control group( P 〈0.05) while the level of CD8^+ decreased markedly than that one( P 〈0.05 ) after treatment, the chnical cure and noticeable rates of the AI group were significandy higher than that of the control group ( P 〈 0. 05 ). Conclution : Severe decline and disbalance of natural kill cell and T lymphocyte immune functional were observed in patients with ACI. The action mechanism of AI on ACI shoud be regulating the balance of natural kill cell and T lymphocyte and relieving the immune response and inflammation damage of centrel nerve system mediated by cytokines, so as to protect neurons from ischernia injury. It is a new connotation for the study of ACI prevention and therapy in the future.
