摘要
目的研究胰头癌患者胰腺外分泌功能的变化及影响因素。方法确定3个试验组分别为对照组:8例患者,慢性胰腺炎(chronicpancreatitis,CP)组:12例患者,胰头癌(pancreaticcancer,PC)组:24例患者。对3组分别进行PFD试验,通过测定尿液中PABA的排泄率来了解3组的胰腺外分泌功能并进行比较,总结出胰头癌患者胰腺外分泌功能的变化特点。同时通过ERCP测定胰头癌患者的胰管直径(x1),超声内镜(EUS)和胰腺CT测肿块的大小(x2),并一步研究胰头癌患者PABA的排泄率(y)与上述变量之间的关系。结果胰头癌患者胰腺外分泌功能明显减低,PABA排泄率为(48.83±15.63)%,明显低于对照组(78.88±15.88)%,而重度胰腺炎患者胰腺外分泌功能(48.5±11.39)%也明显低于对照组。胰头癌患者胰腺外分泌功能与有关因素(胰管直径,胰头肿块大小)成线性关系。并按其对胰腺外分泌功能的影响大小排序为:胰管直径,胰头肿块大小。结论胰头癌患者胰腺外分泌功能明显减低,减低的程度与胰管直径、胰头肿块大小成负相关。其中主胰管直径对胰腺外分泌功能的影响最大,其次为胰头肿块大小。
[Objective] To investigate influencing factors and transformations of pancreatic exocrine function in patients with pancreatic cancer. [Methods] To confirm 3 test groups of control group (8 cases), chronic pancreatitis (12 cases) and pancreatic carcinoma (24 cases). To respectively process PFD test on the three groups, comprehend and contrast the pancreatic exocrine function through measuring the excretion rate of PABA in the urine and summarize the characteristic of the pancreatic exocrine function of pancreatic cancer patients. To measure the diameter of the pancreatic duct in the method of ERCP and the size of the tumor in the method of CT and endoscopic ultrasound. To study further the relationship between the variables above-mentioned and the excretion rate of PABA in the urine of the pancreatic carcinoma patients. [Result] The pancreatic exocrine function of pancreatic cancer lowers obviously, which is obviously lower than the control group (78.88±15.88)%. The pancreatic exocrine function of the severe chronic pancreatitis(48.5±11.39)% is obviously lower than the control group either. The presumptive tests and appraisal of variables show that there is a linear relationship between pancreatic exocrine function and the related factors such as the diameter of the pancreatic duct,the size of tumor. The turn of the influence factors is the diameter of the pancreatic duct, the size of tumor. [Conclusion] The pancreatic exocrine function of pancreatic cancer lowers obviously and the extent has the linear relationship with the diameter of the pancreatic duct, the size of tumor in turn.
