摘要 目的分析放射性脑损伤的MRI表现。方法回顾性分析2000年9月~2005年5月就诊于该科的15例放射性脑病的MRI资料,10患者均因头颈部肿瘤接受过1、2疗程的放疗,MRI资料包括平扫与增强扫描,3例因疑肿瘤复发而经手术确诊为放射性脑损伤。结果急性放射性脑损伤表现为大片状水肿,T1WI上为低信号,T2WWI为高信号,增强后无异常强化。慢性期表现平扫T1WI为大片状低信号,T2WI为大片状高信号,增强后表现为珊瑚状、地图状和环状等不规则形。结论放射性脑损伤表现为水肿与占位效应,慢性期除水肿外,增强T1WI表现为特殊形态的强化。 [Objective] To analyze magnetic resonance imaging (MR]) representations of radiation-induced brain injury. [Methods] MRI imagings of 15 patients who had suffered from radiation-induced brain injury were reviewed. All the patients received 1 or 2 radiotherapy periods during September 2000 to May 2005 because of the head and neck cancers. They underwent MR scan with T2-weighted sequences in the axial plane and Tl-weighted sequences in both axial and sagittal plane. All the patents were also examined with Gd-DTPA enhanced MRI. 6 patents who were suspected recurrence in MRI scans but were confirmed radiation-induced brain injury after excisions. [Results] Acute radiation-induced brain injury shows as large patch edema, which was low signal intensity on T1-weighted imaging and high intensity on T2 weighted imaging. Delayed radiation-induced brain injury manifested edema and space occupying signs, with irregular shapes and sizes enhancedment. [Conclusions] MR] revealed edema of the brain in all radiation-induced brain injuries, during acute phase, no enhancement are seen. But in delayed phase, the edema aggravated, lesions enlarged, with irregulat shapes like coral, map, or ring in enhanced scans.
