摘要
目的探讨高钾血症引起窦室传导的机制、心电图改变以及急诊治疗方法。方法(1)临时心脏起搏治疗;(2)对症药物治疗;(3)血液透析治疗。结果经临时心脏起搏、药物或血液透析治疗,4例血钾及心电图完全恢复正常,1例心电图仍为持续性房颤。结论对高钾引起的窦室传导,尤对伴有缓慢心室率者,给予安装临时心脏起搏器的同时进行对症药物或血液透析治疗,可有效预防恶性心律失常的发生。
Objective To discuss the mechanism and clinical characters of sinoventricular conduction caused by hyperkalaemia and its treatment. Methods The treatment included temporary pacemaker,medication and hemodialysis. Results Five patients were treated by temporary pacemaker, drugs and hemodialysis. The potassium level and ECG recovered to normal in 4 patients,one case had a persistent atrial fibrillation. Conclusion Temporary pacemaker combined with medication therapy or hemodialysis can effectively prevent malignant arrhythmias in the patients of sinoventricular conduction caused by hyperkalaemia, especially with slow heart rate.
