摘要
目的探讨前列腺癌骨转移特征。方法对64例前列腺癌患者行全身骨显像,分析其骨转移发生率,好发部位,病灶分布特点、形态、摄取显像剂情况等。结果骨转移发病率为81.3%,均为成骨型病变,各部位发病率以脊椎(71.9%)、骨盆(64.1%)、肋骨(48.4%)较高。骨转移患者中,多发性转移发病率76.9%。身体左、中、右各部位病灶数目无统计学差异。单发、少发病灶85.7%呈点状,14.3%呈团块状;脊椎病灶占67.9%,骨盆17.9%,胸骨或股骨3.6%,肋骨7.1%;单病灶、二病灶骨转移均产生于脊椎或骨盆。骨转移病灶的发生存在一定伴随关系,98.7%的除脊椎、骨盆之外的病灶伴随脊椎或骨盆转移而存在。结论前列腺癌易发生骨转移,表现为以成骨为主、常始发于脊椎或骨盆、好发于中轴骨、有序性发生、随机性分布、以“点状”为早期病灶形态且常呈多发性多形态病灶并存的特征。
Objective To explore the characteristic of bone metastasis of prostate cancer. Methods Whole body bone imaging was performed for 64 patients with prostate cancer,and the incidence rate of bone metastasis,predilection site,focus distribution, focus forrn,99m^Tc-MDP uptake of focus et al were analyzed. Results The incidence rate of bone metastasis is 81.3%. The incidence rate of the acantha (71.9 % ), pelvis (64.1% ), rib (48.4 % ) is bigger than that of other bone. All focuses are the lesion of osteogenesis, and the incidence rate of more focuses (more than three focuses ) is 76.9% in the patients of bone metastasis. It is not significant difference for the focus number of left,mediacy,right in the whole body. It is 85.7 % of punctation and 14.3 % of lump form in the patients of less than three focuses,and there are 67.9 % focuses in the acantha, 17.9 % in the pelvis,3.6 % in the breastbone or thighbone,7.1% in the rib. All of single focus and two focuses existed either in the acantha or in the pelvis. Different focuses of bone metastasis existed in the concomitance that 98.7% focuses except the focus of the acantha and pelvis accompanied that of the acantha or pelvis. Conclusion The bone metastasis is very common in the patients with prostate cancer. The characteristic of the metastasis focus involve the several aspects that the focus is mainly the osteogenesis lesion and easily infringes the acantha,pelvis or rib and its appearance is in regular sequence and its distribution is random and the form of its forepart focus is punctation and they often simul exist in lots of focuses with different form.
