摘要
为了探讨人类脑干听觉诱发电位(BAEPs)中Ⅲ波及其以后各波的确切神经发生源,对20例大脑半球切除术后远期继发半侧脑干萎缩的病例进行了双侧BAEPs和脑干截面积的研究。结果发现,萎缩侧脑干BAEPs中的Ⅲ波和Ⅴ波潜伏期缩短。此外,萎缩对侧脑干呈代偿性增粗的同时,Ⅴ波波峰绝对潜伏期延长。结果提示:Ⅲ、Ⅴ波的神经发生源位于刺激的同侧。
Thepurposeofthestudywastoprobeintothetrueneuralgeneratorsofthebrain-stemaudiotoryevokedpotentialsinhumansafterhemispherectomy,especialywithregardtopeaktheⅢandⅤwaves.Twentypatientswhohadundergonecerebralhemispherectomybetween1985and1992forthealeviationofintractableepilepsywereelectrophysiologicalystudied.Bilateralbrainstemauditoryevokedpotentials(BAEPs)wereaveragedandmappedoutoverthescalp.Asthelongperiodsafterhemispherectomyentailedmassiveretrogradedegenerationofthesemibrainstemontheoperatedside,itwasfoundthatthelatencyofpeakⅢwasshortenontheoperatedside(P<0.05)andthelatencyofpeakⅤwasdelayedonthecontralateralside(P=0.001).TheresultsofthisstudyindicatedthattheneuralgeneratorsofthepeakⅢandⅤshouldbelocatedinipsolateralclickstimulationandpeakⅢaswelaspeakⅤinhumansmayhavecontributionsfromtheipsolateralcerebralcortex.Besides,itwasconsideredthattheneuralgeneratorsmightberelatedtothevolumeconductor.
