摘要
目的探讨降纤酶联合低分子肝素钙对进展性脑梗死的疗效。方法71例进展性脑梗死患者随机分为治疗组(36例)、对照组(35例),治疗组予降纤酶联合低分子肝素钙治疗,对照组予常规治疗,于治疗前、治疗第3天、第7天、第15天进行神经功能评分和临床疗效检测,治疗前后进行血浆纤维蛋白原(Fib)、红细胞压积检测。结果治疗组第3天、第15天时评分显著低于对照组(P=0．001；P=0．045)；两组在第15天时与治疗前相比较,评分均显著下降(P＜0．01)。治疗组的疗效明显优于对照组,差异有显著性意义(P＜0．05)。两组Fib和红细胞压积与治疗前比较,均显著下降(P＜0．001)；治疗组与对照组相比较,两者均显著下降(P＜0．05)。结论降纤酶联合低分子肝素钙显著降低进展性脑梗死患者的Fib、红细胞压积,改善患者的神经功能。
Objective To observe the effect of batroxobin and low-molecular weight heparin calcium in the treatment of progressive cerebral infarction. Methods Seventy-one cases of progressive cerebral infarction were randomly divided into treatment group （n=36） and control group （n=35）. The treatment group accepted batroxobin plus low-molecular weight heparin calcium and the control group accepted conventional therapy. The nerve dysfunction was judged before and 3, 7, 15 d after treatment. Fib and hematokrit were checked before and after treatment, Results Scores of nerve dysfunction in treatment group were significantly less than those in control group at 3 and 15 d （3 d, P=0,001; 15 d, P= 0.045）. Scores at 15 d in the 2 groups were significantly less than those before treatment （P〈0.01）. Therapeutic effect of treatment group was significantly better than that of control group （P〈0.05）, Fib and hematokrit of all patients were significantly decreased after treatment （P〈0.001）, and the treatment group was significantly lower than the control group after treatment （P〈0.05）. Conclusion Batroxobin and low-molecular weight heparin calcium can significantly degrade Fib and hematokrit and improve nerve function in progressive cerebral infarction patients.
