电化学发光免疫分析法快速测定B型尿钠肽在慢性心力衰竭诊断中的应用 被引量:2

摘要 目的 通过电化学发光免疫分析法快速检测慢性心力衰竭(CHF)患者血中尿钠素(BNP),研究BNP对慢性心力衰竭患者早期诊断、危险分层及左心室功能评价的价值。方法 对98例CHF患者分别以不同病因、心功能分级、超声心动母捡查指标等分组,以电化学发光免疫分析法测定血浆BNP。分析血浆BNP与美国纽约心脏病协会(NYHA)心功能分级及左心室射血分数之间的关系。结果 按不同病因分组可见:不同原发疾病血浆BNP水平差别不明显;按心功能分级:Ⅰ~Ⅳ级患者血浆BNP逐步增加(P〈0.001);超声心动图检查指标:左心室舒张末期内径≤55mm的血浆BNP显著低于内径〉55mm者(P〈0.01),且左心室射血分数≤0.50的血浆BNP显著大于射血分数〉0.50者(P〈0.001)。结论 电化学发光免疫分析法快速测定CHF患者的血浆BNP水平对快速诊断CHF及评价危险分层有较高价值。 Objective To research the value of plasma BNP testing in early diagnosis of chronic congestive heart failure(CHF) and evaluation of the risk stratification and left ventricle function, by quickly detect plasma BNP with electrochemiluminescence. Methods Ninety-eight CHF patients were divided into groups as the different disease results, heart function, ultrasonic echocardiogram,etc. The plasma BNP was detected with electrochemiluminescence and the relation between the plasma BNP and hearf function of NYHA,left ventricle ejection fraction was analysed. Results The differences of the plasma BNP between the different disease results were not obvious. The mean BNP concentration in blood sample obtained from patients with the degree of NYHA from Ⅰ to Ⅳ were increased gradually. The BNP levels in patients with low left ventricular end-diastolic diameter(LVEDD) (≤55mm) were lower than that in the patients with high LVEDD(〉 55mm). (P〈0.01). The BNP levels in patients with low left ventricular ejection fraction(≤0.50) were higher than that inthe patients with high LVEF(〉0.50). (P〈0. 001). Conclusion The method testing plasma BNP of CHF patients with electrochemiluminescence has very high value to quick diagnosis of CHF and evaluation of the risk stratification.
