摘要
目的探讨肿瘤坏死因子-α(TNF-α)在脑出血(ICH)后的作用及七叶皂苷钠对其影响.方法 2004年2月~2005年2月在中南大学湘雅医院选择健康成年雌雄不限SPrague-Dawley(SD)大鼠85只随机分为ICH模型组(MG)20只、治疗组A(TGA)20只、治疗组B(TGB)20只、假手术组(SG)20只和正常对照组(NG)5只,采用胶原酶制作大鼠ICH 模型,通过免疫组化法检测血肿周围脑组织中TNF-α蛋白的表达变化,同时检测基质金属蛋白酶-9(MMP-9)的表达;并检测七叶皂苷钠干预后TNF-α的变化.结果 TNF-α蛋白在NG和SG表达很少;而在MG出血后6h 开始上升,24h 达高峰,以后逐渐下降,96h 时明显减少(P〈0.01);在出血后6~96h TNF-α的蛋白表达量与MMP-9基因 mRNA和蛋白表达呈正相关.在七叶皂苷钠治疗组TNF-α的蛋白表达量明显受到抑制,特别是TGB抑制效果更明显,并存在一定的量效关系,呈负相关(P〈0.01).结论 ICH后 TNF-α的表达水平提高可能有协同诱导MMP-9的表达作用,从而参与了血管源性脑水肿的形成;而七叶皂苷钠抗脑水肿的作用可能通过包括对TNF-α的抑制在内的多种途径来完成的.
Objective To observe the expression of tumor necrosis factor-α(TNF-α) in rat brain tissue in the border zone of the hematoma and explore the relationship between the expression of TNF-α and sodium aescinate after intracerebral hemorrhage(ICH). Methods From February 2004 to February 2005,85 heathy adult SD rats were divided randomly into:intracerebral hemorrhage model group (MG)(20),sodium aescinate treatment group A ( TGA ) ( 20 ), sodium aescinate treatment group B (TGB) ( 20 ), sham-operated group (SG) ( 20 ) and normal contral group(NG) (5)in Xiangya Hospital of Zhongnan University. Rat model of ICH was established by injection of collagenase into rat caudate nucleus. Immunohistochemistry was respectively used to evaluate TNF-α and MMP-9 protein expression. The expression of MMP-9 mRNA in the ICH rat brains was investigated by in situ hybridization. The changes of TNF-α after sodium aescinate was detected on ICH. Results The expression of TNF-α was little in NG and SG. However,the expression began to rise at 6h around the hematom of the operation side ,and the peak was at 24h in MG. Afterwards it began to reduce slowly,and it was less at 96h(P〈0.01). After 6-96h,there was a positive correlation with the expression of TNF-α protein and the expression of MMP-9 protein and mRNA. In TGA and TGB,the expression of TNF-α was inhibited transparently,especially in TGB that there was a negative dose-effect relationship (P〈 0.01 ). Conclusion After the injuring course of ICH, the increased expression of TNF-α was suggested that it plays an important role in the pathological course of ICH. Also sodium aescinate can inhibit the expression of TNF-α significantly.
