摘要
目的 了解新建大型水库后居民肠道寄生虫感染状况及影响因素,为该地区肠道寄生虫病防治提供依据。方法 选择三峡建坝后的重庆市万州较典型的地区,调查当地居民生活行为方式并进行寄生虫病病原学检查。结果 当地居民饮用水来源主要为河水(占68.7%),手压井水占7.7%,自来水占23.6%;使用无害化厕所的比例为13.5%,86.5%的住户使用室内简易厕所;居民寄生虫感染率为45.4%,男、女感染率分别为42.9%和48.0%。检查出5种肠道寄生虫,其中蛔虫、姜片虫、钩虫、鞭虫、蛲虫感染率分别为10.9%、26.2%、16.9%、3.3%和0.3%。感染1种、2种、3种虫者分别占感染人数的71.7%,23.5%和4.8%。蛔虫感染率10~14岁年龄组最高,姜片虫感染率30~39岁年龄组最高,钩虫感染率30~59岁年龄组最高。结论 三峡水库建成后,居民的卫生状况未能得到彻底改善,肠道寄生虫病仍然是主要危害当地居民健康的疾病之一。因此应加强对这类地区的寄生虫病监测,并通过健康教育的方式改变居民的生活环境及不良的健康行为。
Objective To know the infection status and influence factors of intestinal parasites among population after the newly-buih Three Gorges Reservoir area and provide information for control of intestinal parasitic diseases in this area. Methods Wanzhou District in Chongqing Municipality was chosen as the pilot for the investigation after the construction of Three Gorges Dam. Production, life style and behavior of the residents were surveyed and fecal examination of parasites was carried out. Results The main source of drinking water there is river (68.7%), hand-pressed wells (7.7%) and tap water (23.6%). In this area, hygienic lavatories is only used by 13.5% households, coarse lavatories without the function of killing parasitic eggs and bacilli were used by the rest majority (86.5%). Total prevalence of the intestinal parasites was 45.4%, with 42.9% in males and 48.0% in females. Five kinds of intestinal parasites were detected by fecal examination. Among them the prevalence rate of ascaris, fasciolopsis, hookworm, trichuris and enterobius was 10.9%, 26.2%, 16.9%, 3.3% and 0.3%, respectively. People that infect one, two and three kinds of parasites accounted for 71.7%, 23.5% and 4.8%, respectively. The age group with the highest infection rate of ascaris was from 10 -14 years of age, fasciolopsis, 30-39 years, and hookworm, between the age of 30 and 59 years. Conclusion After the construction of Three Gorges Reservoir, although a distinct change on environment has shown, the sanitation has not been improved as a whole. And, as a result, intestinal parasitic diseases are still one of the main diseases causing morbidity of the residents. So surveillance and control of the parasitic diseases in the area need to be strengthened, especially health education should be provided to the local residents so as to change their unhealthy life style as well as behavior.
