摘要
【目的】观察中药薰蒸结合穴位按摩对产后关节痛的治疗效果。【方法】采用随机数字表法将90例产后关节痛患者随机分为穴位按摩组、中药薰蒸组和中药薰蒸结合穴位按摩组(简称薰蒸合按摩组),每组各30例。中药薰蒸组用当归、防风、羌活、宽筋藤制成的粉剂加入热水中薰蒸双下肢和热敷四肢的关节部位,穴位按摩组给予上、下肢手法按摩和穴位(曲池、内关、外关、合谷、足三里、三阴交等)按压,薰蒸合按摩组既给予穴位按摩组相同的按摩治疗,又给予中药薰蒸组相同的中药薰蒸治疗,3组疗程均为7 d。治疗前后进行关节痛程度计分比较,观察临床疗效,并随访42 d。【结果】穴位按摩组总有效率为80.00%,中药薰蒸组的总有效率为83.33%,薰蒸合按摩组的总有效率为96.67%,3组比较,穴位按摩组与中药薰蒸组的疗效相仿(P>0.05),薰蒸合按摩组疗效优于中药薰蒸组和穴位按摩组(P<0.05)。3组治疗后关节痛程度均较治疗前有显著性改善(P<0.05或P<0.01),穴位按摩组和中药薰蒸组在改善关节痛程度方面疗效相仿(P>0.05),而薰蒸合按摩组的疗效优于中药薰蒸组和穴位按摩组(P<0.05)。42 d后复查或回访,薰蒸合按摩组没有1例疼痛加重,穴位按摩组4例疼痛加重,中药薰蒸组3例疼痛加重,薰蒸合按摩组与其他两组比较差异均有统计学意义(P<0.05)。【结论】中药薰蒸结合穴位按摩治疗产后关节痛的疗效优于单纯中药薰蒸或穴位按摩,说明中药薰蒸和穴位按摩在治疗产后关节痛中具有协同作用。
[Objective] To observe the therapeutic effect of fuming and steaming with herbal medicine and acupoint massage for postpartum arthralgia （PA）. [Methods] Ninety PA patients were equally divided into 3 groups by using the random number table. Group A was treated by massaging four limbs and by pressing acupoints of Quchi, Neiguan, Waiguan, Hegu, Zusanli and Sanyinjiao. Group B was treated by fuming and steaming the lower limbs with the powder of Radix Saposhnikoviae, Rhizoma et Radix Notopterygii and Caulis Trachelospermi, and by hot-compressing on joints of the four limbs. Group C was treated by the combined therapy of groups A and B. The three groups were treated for 7 days. The arthralgia scoring was compared before and after treatment, and the therapeutic effect was observed after treatment. The patients were followed-up for 42 days. [Results] The total effective rate was 80.0% in group A, 83.3% in group B and 96.7 % in group C. The effect was similar in groups A and B （ P 〉 0.05）, and was superior in group C than that in groups A and B （ P 〈 0.05）. Arthralgia was much relieved in the three groups after treatment as compared with that before treatment （P 〈 0.05 or P 〈 0.01）. Arthralgia relief was similar in groups A and B （ P 〉 0.05）, and was superior in group C than that in groups A and B （ P 〈 0.05）. The result of follow-up showed that arthralgia became severe in none of group C, in 4 patients of group A and in 3 of group B, the difference being significant between group C and the other two groups （P 〈 0.05）. [Conclusion] The combination of fuming and steaming with herbal medicine and acupoint massage exerts a better effect for postpartum arthralgia than any of the above two, indicating that the combination of the above two has synergistic action.
