糖足方治疗糖尿病足及其对血浆NO、ET、VEGF的影响 被引量:5

Effect of Tangzu Prescription for the Treatment of Diabetic Foot and Its Influence on Plasma NO,Endothelin and Vascular Endothelial Growth Factor

摘要 【目的】探讨以糖足方(主要由黄芪、生地黄、当归、川牛膝、莪术、玄参、虎杖等中药组成)为主的综合疗法治疗糖尿病足(DF)的临床疗效及其对内皮功能的影响。【方法】将108例DF患者随机分为糖足方组和西药组,每组54例,糖足方组给予糖足方治疗,西药组给予654-2治疗,对照观察DF患者溃疡肉芽组织出现时间(GT)、溃疡愈合时间(HT),比较两组临床疗效及治疗前后患者血浆一氧化氮(NO)、内皮素(ET)、血管内皮生长因子(VEGF)水平的变化,并设立35例健康体检者作为正常对照组。【结果】糖足方组GT、HT均短于西药组(P<0.05或P<0.01),愈合率高于西药组,截趾(肢)率低于西药组,两组比较,糖足方组疗效优于西药组(P=0.0035)。治疗后西药组血浆NO水平增高(P<0.01),ET、VEGF水平无显著性变化(P>0.05);糖足方组血浆ET、VEGF水平降低,NO水平增高(P<0.01),与西药组比较血浆ET、VEGF及NO均有显著性差异(P<0.05或P<0.01)。【结论】以糖足方为主的综合疗法治疗DF的效果及其对血管内皮的保护作用优于以654-2为主的综合疗法。 [Objective] To investigate the therapeutic effect of Tangzu Prescription (TP), mainly composed of Radix Astragali, Radix Rehmanniae, Radix Angelicae Sinensis, Radix Achyranthis Bidentatae, Rhizoma Curcumae, Radix Scrophulariae, Rhizoma Polygoni Cuspidati, etc., for the diabetic foot (DF) and to observe its influence on the endothelial function. [ Methods] One hundred and eight DF patients were equally randomized into two groups: on the basis of routine basic treatment and surgical treatment, group A was treated with TP and group B with 654-2 additionally. Thirty-five healthy volunteers served as the normal controls. Granulation-formation time (GT), healing time (HT) of ulceration, therapeutic effects were compared in the two groups after treatment. The changes of plasma levels of nitric oxide (NO), endothelin (ET) and vascular endothelial growth factor (VEGF) were observed before and after treatment. [Results] GT and HT were shorter, healing rate higher and amputation rate lower in group A than those in group B, indicating that the therapeutic effect was better in group A than that in group B ( P = 0.0035). After treatment, plasma NO level was increased in the two groups ( P 〈 0.01 ), ET and VEGF remained unchanged in group B but decreased in group A ( P 〈 0.01 ). The differences of ET, VEGF and NO were significant between the two groups ( P 〈 0.05 or P 〈 0.01 ). [ Conclusion ] The comprehensive therapy including TP has a better effect for DF and in improving endothelial function than the therapy including 654-2.
