血小板相关抗体直接法与间接法的比较 被引量:1

摘要 目的:比较直接法与间接法血小板相关抗体(PAIg)分析的诊断意义。方法:使用流式细胞术,同时用直接法与间接法分析健康人组、免疫性血小板减少性紫癜患者组和非免疫性血小板减少性紫癜患者组的血小板相关抗体,并对检测结果作比较分析。结果:直接法分析诊断免疫性血小板减少性紫癜的敏感度为61.02%,特异度为93.94%,阳性预测值为94.74%,阴性预测值为57.41%。间接法分析诊断免疫性血小板减少性紫癜的敏感度为66.10%,特异度为87.88%,阳性预测值为90.70%,阴性预测值为59.18%。同时使用直接法和间接法分析、诊断免疫性血小板减少性紫癜的敏感度为84.75%,特异度为87.88%,阳性预测值为92.59%,阴性预测值为76.32%。结论:同时使用直接法和间接法PAIg分析,可有效提高血小板相关抗体检出率。 Objective To compare the diagnostic significance of direct and indirect platelet-associated immunoglobulin (PAIg) analysis. Methods Direct and indirect analysis of PAIg by flow cytometry was performed in healthy group, autoimmune thromhocytopenic purpura group and non-autoimmune thrombocytopenic purpura group. The results were statistically analyzed. Results Direct PAIg analysis for autoimmune thrombocytopenic purpura had the sensitivity of 61.02%, the specificity of 93.94%, the positive predicative value of 94.74%, and the negative predicative value of 57.41%. Indirect PAIg analysis for autoimmune thrombocytopenic purpura had the sensitivity of 66.10%, the specificity of 87.88%, the positive predicative value of 90.70%, and the negative predicative value of 59.18%. The combination of direct and indirect PAIg analysis for autoimmune thrombocytopenic purpura had the sensitivity of 84.75%, the specificity of 87.88%, the positive predicative value of 92.59%, and the negative predicative value of 76.32%. Conclusions The combination of direct and indirect PAIg analysis may greatly improve the diagnostic efficiency of autoimmune thromhocytopenic purpura.
