PTEN、MMP-7在骨巨细胞瘤中的表达及其意义

摘要 目的探讨骨巨细胞瘤中的PTEN、MMP-7表达与临床分期分级及复发转移的关系。方法采用免疫组化法检测65例骨巨细胞瘤标本PTEN、MMP-7基因蛋白的表达情况,分析其与骨巨细胞瘤临床病理分期、分级及复发与转移的关系。结果PTEN、MMP-7的阳性表达率分别为57.0%、72.3%,其中在MMP-7阳性表达组中PTEN的阳性表达率为30.0%,在MMP-7阴性表达组中,PTEN的阳性表达率为48.5%,二者呈负相关;在“三结合分类”组中PTEN的阳性表达率分别为83.3%、68.2%、28.0%,MMP-7阳性表达率分别为39.0%、72.5%、96.0%,组间差异明显,(P<0.05);复发组中PTEN、MMP-7的阳性表达率分别为33.3%、86.3%,无复发组中PTEN、MMP-7的阳性表达率分别为64%、68%,两组间差异明显(P<0.05),而与病理分期、分级及年龄、性别无统计学差异。结论PTEN、MMP-7的表达与“三结合分类”有一定的关系,并与转移,复发密切相关,联合检测有助于骨巨细胞瘤的临床评估,对其预后判断具有重要临床意义。 Objective To evaluate the expressions of phophatase and tensin homolog deleted on chromosome 10 (PTEN) and matrix metallo-proteinases-7 (MMP-7) in giant cell tumor of bone (GCT) and their relationships with pathological types and metastasis and recurrence. Methods The expressions of PTEN and MMP-7 in 65 cases of human GCT were detected by using immunohistochemical staining. The expressions of PTEN and MMP-7 and metastasis and recurrence of the tumor were analyzed. Results The expressions of the PTEN and MMP-7 in GCT were 57.0% and 72.3% respectively. The expression of PTEN in the MMP-7 positive group and negative were 30.0% and 48.5%. The expressions of PTEN between MMP- 7 positive and negative groups were significantly different, P〈0.05 (P=0.001). In Ennecking-Jeffe - Campanacci groups, the expressions of PTEN and MMP-7 were 83.3% and 68.2% and 28.0% and 39.0% and 72.5% and 96.0% respectively. Each of them expressed significantly differently, P〈0.05 (P=0.046, 0.001 ). The expressions of PTEN and MMP-7 in the recurrence group were 33.3% and 86.3%, but in the nonrecurrence group were 64% and 68%, their expressions were statistically related to the recurrence of GCT, P〈0.05 (P=0. 035, 0.006). However, the expressions of PTEN and MMP-7 had no statistically significance between the pathological types. Conclusions The decreased expression of PTEN and increased expression of MMP-7 have significant relationship to the prognosis of GCT. The combined detection of PTEN and MMP-7 may be of important clinical value to evaluate the infiltrative ability and prognosis of GCT.
