摘要
为探讨累及舌根之会厌癌的手术治疗并恢复喉功能的方法，对２６例进行了回顾性研究。其中男性１４例，女性１２例，年龄从４３岁至６２岁，除２例行全喉切除外，２４例行上半喉或水平垂直次全喉加舌根切除，将下半喉上吊与咽及舌根相吻合。１１例行双侧颈廓清术，１０例行单侧颈廓清术。结果２４例术后均能说话，２０例拔除气管套管，喉功能全部恢复，５年生存率５７．９％（１２／１９）。认为单纯上半喉加舌根切除，咽进路更合理。术中注意保留舌下神经，将下半喉尽力向前上提。
casesoftheepigloticcancerextendedtothebaseofthetonguewerestudiedretro-spectively.Amongthem,14weremaleand12female.Theagevariedfrom43to62.Besidestwocaseswhounderwenttotallaryngectomy,theothertwenty-fourweretreatedwithsupraglotichori-zontalpartiallaryngectomyorhorizontoverticalsubtotallaryngectomyandresectionofpartofbaseofthetongue.11casesreceivedbilateralradicalneckdissection(RND)and10casesunilateralRNDsi-multenously.Theresultsshowedtwenty-fourcaseshadresumedspeakingability,twentyhadbeendecannulatedandrestoredthelaryngealfunctions.The5-yearsurvivalratewas57.9%(12/19).Itwasconcludedthattranspharyngealapproachwasreasonableforsupraglotichorizontalpartiallaryn-gectomyassociatedwiththeresectionofthebaseofthetongue.
