摘要
目的:探讨血清CA19-9对胆囊癌的诊断及疗效判断价值。方法:采用化学发光法对胆囊癌、胆囊良性病变及良性非胆系疾病患者共60例进行手术前后血清CA19-9检测。结果:胆囊癌患者术前血清CA19-9水平明显高于胆囊良性病变者(P<0.01),检测的敏感性为80.0%,特异性为97.5%。胆囊癌根治性手术后血清CA19-9水平较术前有显著降低(P <0.01),而姑息性手术前后无显著变化。结论:手术前后血清CA19-9水平的检测,对胆囊癌的诊断及疗效判断具有较高的临床应用价值。
Objective:To investigate the value of serum carbohydrate antigen 19-9 (CA19-9) level in the diagnosis and treatment of gallbladder cancer patients. Methods.. Serum CA19-9 was measured by using chemiluminescence ashy before and after operation in 20 patients with benign gallbladder disease, 20 patients with malignant gallbladder disease, and 20 patients without gallbladder disease. Results: The preoperative serum CA19-9 level in patients with gallbladder cancer was significantly higher than that of benign gallbladder diseases (P〈0.01). The sensitivity and specificity of preoperative serum CA19-9 level for the diagnosis of gallbladder cancer was 80. 0% and 97. 5%, respectively. The serum CA19-9 level after tumor radical resection in patients with gallbladder cancer significantly decreased from preoperative to postoperative(487. 9 ± 542. 5μ/ml vs. 130. 1 ± 136. 1 μ/ml ,P〈0. 01 ), whereas the serum CA19-9 level did not significantly changed after tumor palliative resection compared with that of pre-operation. Conclusion.. Serum CA19-9 level is an effective tumor marker in diagnosing gallbladder cancer and deciding whether the tumor has been radically resected.
