摘要
目的分析创伤性膈疝的发病因素,早期诊断方法,误诊漏诊的原因及治疗的方法。方法回顾性总结1992-07～2005.05我院收治的23例创伤性膈疝病人的临床病例资料。结果伤后24h内确诊8例,术前确诊15例,78%的患者合并其他脏器损伤,采用剖胸手术14例,单纯胸腔镜手术3例,胸腔镜辅助剖腹手术3例,剖腹手术2例,无胸腹联合切口或剖胸并剖腹手术病例。治愈22例,死亡1例,死亡率4.3%。结论创伤性膈疝临床表现不典型,多有合并伤,易误、漏诊,辅助检查以胸部X线为首选,手术方式视具体病情而定,早期诊断、及时手术是提高治愈率、降低死亡率的关键。
Objective To analyze the cause, early diagnostic methods, misdiagnostic cause and of the traumatic diaphragmatic hernia, Methods The clinical records of 23 cases with diaphragmatic hernia admitted to our hospital between July 1992 to May 2005 were reviewed treatment traumatic Results The positiue diagnosis was made in 24 hours after injury in 8 casesana maae preuperatively Associated injuries with other viscera occurred in 78% of the cases. Thoracotomy were performed in 14 cases, VITS in 3 cases, VATS together with laparotomy in 3 cases, Laparotomy in 2 cases. There was no tharacoabdominal incision or tharacotomy together with laparotomy. 22 cases were cured and 1 case was dead. Conclusions There areict typical symptoms of traumatic diaphragmatic hernia and associated injuries occur mostly so that it tends to be misdianosed. Chest X-ray is the first technigue of examination to choose. Early diagnosis and treatment is impartant to improve cure rate and decrease martality.
