摘要
目的为探讨动态心电图安装起搏器术后功能监测的价值。方法采用美国通用公司生产的三通道24小时动态心电图(Holter)对26例(36例次)安装心脏起搏器患者进行随访。结果与讨论发现4名患者共5例次出现起搏障碍,其中2名患者同时出现起搏和感知障碍,距安装起搏时间8～9年,另两名患者距起搏器安装时间分别为7天、36天,起搏功能障碍,感知功能良好,1例心房电极起搏心室,心室电极起搏无效,经X照片证实心房电极移位至右室流出道,另一名患者频繁出现起搏障碍,X照片证实心室电极移位至右室流出道,行电极重置术后仍出现4次无效起搏,结合临床情况分析为起搏阈值升高所致,动态心电图能及时发现起搏障碍,检出导线移位,提出警示,在起搏器术后随访中起很重要的作用,应该作为起搏器术后病人的常规检查。
Objective For follow-up of the function of pacemaker. Methods 26 patients received the there leads 24-hour ambulatory electrocardiogram（Holter）monitoring that made by GE USA. Results Four patients had pacing dysfunction, of which two patients also had sensing dysfunction after 8 or 9 years after pacemaker operation. The other two patients had pacing dysfunction after 7 or 36 days after operation. One patient＇s atrial electrode was pacing the ventricular and ventricular electrode didn＇ t work, It was proved that the atrial electrode shifted to the right ventricular outflow path. The last patient＇s ventricular electrode shifted to the outflow path ,after rejusted the ventricular electrode ,the patient still had pacing sysfunction four times ,It is related to the higher threshold . Conclusion Pacemaker up with Holter monitoring for assessing the signifiance of transient sensing and pacing dysfunction of pacemaker is useful.
