摘要
目的分析桡骨远端骨折后腕部功能与远侧桡尺关节(DRUJ)稳定性之间的关系,探讨桡骨远端骨折影响DRuJ稳定性的原因。方法对54例经保守治疗的桡骨远端骨折患者进行8~10个月(平均9个月)的随访,每例患者都进行放射学评估和DRUJ稳定性检查,采用改良Gartland and Werley评分系统对腕部进行功能评估。结果桡骨远端骨折后DRUJ不稳17例。DRUJ不稳的患者有更差的GW评分,但DRUJ稳定与DRUJ不稳两组患者总GW得分间差异无显著性意义(P〉0.05),尺骨茎突骨折对远侧桡尺关节的稳定性无明显影响。结论明显成角或短缩畸形的桡骨远端骨折损伤三角纤维软骨复合体(TFCC)可能是造成DRUJ不稳的主要原因,伴随桡骨远端骨折的尺骨茎突骨折对DRUJ稳定性无明显影响。
Objective To analyze the relationship between wrist function following distal radial fracture and the stability of distal radio - ulnar joint stability ( DRUJ), to assess the reason affecting the stability of DRUJ after distal radial fracture. Methods Totally 54 patients with distal radius fracture were treated non - surgically and followed up for mean 9 (8 - 10) months, All patients had their wrist joints and DRUJ detected for their stability and undergone functional and radiographic evaluation by Gartland and Werley scoring system. Results There were 17 patients with DRUJ instability, patients with DRUJ instabihty following distal radial fracture had a worse Garland and Werley score. No significant difference was found regarding Gartland and Werley score between 2 groups ( P 〉 0. 05). With the ulnar styloid fracture has no obvious impact to DRUJ stability. Conclusion Injury to the triangle fibrocartilagious complex following the distal radial fracture with obvious displaced deformity may be the main cause of the instability of DRUJ, while the ulnar styloid fracture following distal radial fracture has no obvious impact to DRUJ stabihty.
