摘要
目的建立目标病人在疾病的诊治和动态管理过程中涉及的医疗机构之间如何健全与双向转诊有关的可持续运行的人员配备、组织网络、收费标准、联系方式、快捷通道、制约机制等一系列规章制度。方法在宁波市江东区(新城区)和江北区(老城区)各选择一家三级综合医院和一家社区卫生服务中心作为试点单位,以试点单位就诊的社区慢性病病人、精神病病人、残疾人、急诊病人等作为目标病人。成立双向转诊的“中介机构”和“专家会诊组”,按课题拟达到的目标进行跟踪研究。结果建立了一系列与社区卫生服务双向转诊制度相关的各项规章制度,并运行良好。结论虽然建立了一系列与社区卫生服务双向转诊制度相关的各项规章制度,并运行良好,但可持续的运行保障和制约机制还有待于进一步完善。
Objective To establish a set of bylaws on sustainable staffing, organization network, charging standards,contact ways, green channels, and constraint mechanism involved in dual referral between medical establishments in the medical service and dynamic supervision for target patients. Methods A third -level hospital in Jiangdong District （the New District） and a community health care center in Jiangbei District （the Old District） of Ningbo were chosen as experimental settings, where the community patients with chronic diseases, mental diseases, disability, and the emergency patients were chosen as target patients. A dual referral agency and an ＂ Expert Consultation Group＂ were set up to carry out a longitudinal study according to the assigned purpose. Results A set of bylaws concerning the dual referral system of the community health care service was set up and functioned well. Conclusion Despite the successful establishment and good function of the bylaws, a mechanism of safeguard and constraint for sustainable functioning needs further development.
