乳腺癌的抗血管和淋巴管生成治疗

摘要 乳腺癌是一个世界范围内的严重危害妇女健康的恶性肿瘤,在我国其发病率不断上升。但乳腺癌总体治愈率近10年来没有得到显著提高。目前临床治疗效果略有改观的主要原因是由于早期发现、早期诊断、早期治疗,而不是治疗方法本身进步。治疗失败的主要原因是肿瘤的复发和转移。肿瘤的生长、侵袭和转移依赖于血管和淋巴管的生成。对乳腺癌等上皮组织起源的肿瘤而言,淋巴道播散是首要的转移途径。乳腺癌肿瘤存在高微血管密度和高微淋巴管密度,抑制肿瘤的微血管和微淋巴管生成即可抑制肿瘤的生长、侵袭、复发和转移。人们较早发现微血管标记物,乳腺癌的抗血管生成治疗研究较早,现已经有30多种抗血管生成药物,内皮抑素是目前最强的血管生成抑制因子;最近人类才发现podop lan in等淋巴管特异性标记物,因而对乳腺癌的抗淋巴管生成治疗研究还处于起步阶段。可以预测,在不久的将来,以抗肿瘤血管和淋巴管生成治疗为主的生物治疗方法将为乳腺癌的综合治疗带来一次新的飞跃。
