摘要
目的:探讨高血压病、冠状动脉粥样硬化性心脏病(冠心病)的胰岛素抵抗及其与红细胞钙调控的关系。方法:本文对17例高血压病患者、16例高血压病合并冠心病患者的胰岛素抵抗和红细胞内钙调控进行研究,并与16例正常血压者对照。用氧化酶法、放射免疫法观察餐后2小时血糖和胰岛素变化。用放射性同位素45Ca2+示踪技术进行Ca2+通透红细胞膜的流量调控分析,测定人红细胞钙离子通道和人红细胞钙离子泵。结果:17例高血压病患者的胰岛素抵抗水平(IAI)为-4.07±0.62,与16例正常血压者有显著差别(P<0.01)。17例高血压病患者的红细胞钙离子通道水平〔5.73±1.94mmol·(mlcel)-1/30min〕高于16例正常血压者,但无统计学意义。16例高血压病伴冠心病患者的胰岛素抵抗水平为-4.76±0.54和红细胞钙离子通道为7.60±2.61mmol·(mlcel)-1/30min,显著高于17例高血压病患者和16例正常血压者(P<0.05~0.01)。红细胞内钙水平与餐后2小时血胰岛素水平及收缩压呈明显正相关。结论:高血压病合并冠心病患者的胰岛素抵抗现象及红细胞钙离子通道增高,均较单纯高血压病患者更为显著,至?
Objective:To study Insulin resistance(IR) and its relationship with intraerythrocyte calcium regulation in essential hypertension and coronary heart disease. Methods:IR and intraerythrocyte calcium regulation in 17 essential hypertension patients(EH) and 16 EH with coronary heart disease(EH+CHD) and 16 normotensive subjects (NT) were studied.Plasma glucose concentration was measured using oxidase and insulin INS with radioimmunoassay.Erythrocyte calcium channel(ECC) and erythrocyte calcium pump(ECP) were estimated by the analysis of 45 Ca 2+ crossing the erythrocyte membrane using 45 Ca 2+ tracing technique. Results:Significant difference was found between the index of insulin resistance of EH(IAI:-4 07±0 62) and NT( p <0 01).The level of ECC 5 73±1 94 mmol·(ml cell) -1 /30 min was higher in EH than that in NT,but with no statistical significance.The levels of IR (IAI-4 76±0 54) and ECC 7 60±2 61 mmol·(ml cell) -1 /30 min were significantly higher in EH+CHD than those in EH and NT ( p <0 05-0 01).ECC showed significantly positive correlation with INS 2 h.after meal and SBP. Conclusion:The increases of IR and ECC were more marked in EH+CHD than in ET.The relationship between IR and ECC as well as the significance in EH+CHD is worth further study.
