摘要
目的探讨溶血对血清胰岛素测定的影响程度及相应的处理方法。方法采用免疫化学发光法检测两种不同程度的溶血标本以及溶血标本在加入酶抑制剂EDTA后分别在4℃、25℃温度下胰岛素浓度的变化情况。结果轻度溶血标本(Hb 5g/L)胰岛素浓度在25℃温度下3h内下降至原来的30%,在4℃温度下3h内下降到原来的84%。重度溶血标本(Hb 20g/L)胰岛素浓度在25℃温度下3h内下降至原来的4%,在4℃温度下3h内下降到原来的35%;溶血标本(Hb 5g/L)在加入酶抑制剂EDTA后,胰岛素浓度在25℃温度下3h内下降至原来的87%,在4℃温度下3h内基本不下降。结论溶血程度越高,胰岛素降解越快。低温及酶抑制剂EDTA可明显改善溶血对胰岛素测定结果的影响程度。因此,要测定胰岛素的血清需进行低温保存或加入酶抑制剂EDTA。
Objective To discuss the influence of hemolysis on determination of insulin and the treatment method. Methods Two different kinds of hemolytic specimens (Hb 5g/L and Hb 20g/L) pretreated and post- treated by EDTA were tested respectively at 4℃ and 25℃ in different time using chemiluminescence to probe the influence of hemolysis on the measurement of insulin. Results Insulin in slighter hemolytic specimens (Hb 5g/L) at 4℃ and 25℃ was declined to 30% and 84%, respectively, whereas Insulin in heavier hemolytic specimens (Hb 20g/L) was declined to 4% and 35%, respectively, at 25℃ and 4℃ within 3 hours. Insulin in hemolytic specimens (Hb 5g/L) treated by enzyme inhibitor EDTA was declined to 87% at 25℃, and did not change at 4℃ within 3 hours. Conclusion The higher the hemolysis is, the faster the insulin in serum is degraded. Low temperature and enzyme inhibitor EDTA can distinctly reduce the influence of hemolysis on determination of insulin. For determination of insulin, the serum be kept in low temperature or treated with EDTA.
