摘要
目的探讨AO三叶形钢板及May解剖型钢板治疗Pilon骨折的手术适应证及疗效。方法对35例采用AO三叶形钢板及33例May解剖型钢板治疗的Pilon骨折病例进行回顾性分析。其中59例一期内固定；9例行外固定支架固定，2～3周后改用AO三叶形钢板或May解剖型钢板内固定。所有病例均不采用任何外固定，术后第1天即开始功能锻炼。结果随访4～22个月，平均7．1个月，按Mazur等制定的评分系统，临床疗效优40例，良19例，可5例，差4例，优良率86．8％。皮肤坏死4例，伤口感染1例，骨折延迟愈合2例，创伤性关节炎4例。结论AO三叶形钢板及May解剖型钢板是治疗Pilon骨折较理想的固定材料，根据骨折类型和软组织条件，灵活选择手术时机及固定钢板是取得良好手术效果的关键。
Objective To explore the operative indication and curative effect of using AO cloverleaf plate and May anatomic plate to treat Pilon fracture. Methods To review the Pilon fracture cases,in which 35 were treated with AO cloverleaf plate and 33 were treated with May anatomic plate. Among these cases, 59 were done with the one-staged therapy and internal fixation;The other 9 were done with external fixation first, changed to use internal fixation with AO cloverleaf plate and May anatomic plate two or three later. Post-operation,all the cases used any external fixation and started function exercise in the first day. Results The follow-up period was from 4 to 22 months, the average time was 7.1 months. According to Mazur＇s criterion,the result of the treatment was evaluated as excellent in 40,good in 19,fair in 5 and bad in 4,recrosis of skin in 4 cases,wound infection 1 case,delayed union of facture 1 case,traumatic arthritis 4 cases. Conclusion AO cloverleaf plate and May anatomic plate are indeal material for treatment of Pilon fracture. It is important to have the right choice in timing of surgery and correct method of fixation in fracture type and parenchyma condition for a satisfactory result.
