体外冲击波结合红骨髓移植治疗骨不连的临床研究 被引量:3

Clinical study of extracorporeal shock wave therapy with autograft of bone marrow for bone nonunion

摘要 目的研究体外冲击波疗法(ESWT)结合红骨髓移植治疗骨折延迟愈合和骨不连的疗效。方法局麻下自髂骨抽取自体红骨髓约10~15ml,在X线透视下进行严格的骨髓移植操作,同时配合ESWT治疗19例骨不连患者,与单纯ESWT治疗组23例进行对比分析。结果骨髓移植后随访1年,有16例长骨骨不连患者获得愈合,骨折愈合率84,2%,略高于单纯ESWT组;联合治疗组的骨折平均愈合时间为19.13周,比对照组短(P〈0.05)。结论自体骨髓移植后成骨作用明显;采用ESWT结合红骨髓移植治疗骨折延迟愈合和骨不连的方法简单、有效,不失为骨不连非手术疗法中的理想选择。 Objective To study the effect of extracorporeal shock wave therapy with percutaneous autogenous bone marrow grafting in the rearrangement of long bone nonunion. Methods After success in marrow aspiration from patients' iliac crest, 19 eases of patients accepted the treatment of bone marrow grafting and ESWT. Then, it's therapeutic ef- fect was compared with 23 cases of single ESWT group,such as average rate of bone union and average healing time during foUow-up period. The data of this study was analyzed by independent-samples t test,and a P value of 〈0.05 was considered significant. Results 42 cases in both groups were foUowed up for 12 month after therapy. Average rate of bone union was 84.2 % in grafting group and 82.6 96 in single ESW therapy group, average healing time of grafting group was 19.13 weeks, which was shorter than that of control group( P 〈 0.05 ). Conclusion Autograft of bone marrow for bone nonunion is effective, which can promote osteogenesis. Therefore, ESWT with autograft of bone marrow is a kind of optimal therapeutic methods in bone nonunion after fracture.
