摘要
目的 观察中药复方皂矾丸联合西药康力龙、人血丙种球蛋白和环孢素治疗儿童慢性再生障碍性贫血的疗效和不良反应。方法 36例患儿采用中西医结合四药联用:(1)复方造矾丸,3~7粒,2次/d口服,疗程至少6个月以上;(2)康力龙0.1,0.2mg/(kg·d),3次/d口服,疗程至少12个月以上;(3)人血丙种球蛋白1g/kg,1次/月静滴,共6次;(4)环孢素5~8mg/(kg·d),分早晚两次口服,疗程至少6个月。结果:治疗3个月时血常规WBC、RBC、HB、网织红细胞较用药前有所上升,PLT上升不明显;6个月、12个月时所有患儿血常规各项指标均值较用药前、用药3个月时有明显提高,但PLT上升缓慢,治疗至12个月时基本治愈10例(28.6%)、缓解12例(34.3%)、明显进步9例(25.7%)、无效5例(11.4%),总有效率88.6%,未见严重不良反应。结论:中药复方皂矾丸联合西药康力龙、人血丙种球蛋白和环孢素是一种治疗儿童慢性再生障碍性贫血疗效较好和不良反应少的方法。
Objective:To investigate the clinical efficacy of the combinational therapy of Chinese herb of fufang zaofan pill and western drugs of androgen, intravenous immunoglobuiin and cyclospmine in childhood chronic aplastic anemia, and to examine the adverse reaction in the treatment process. Methods: The 36 trial children were treated with four combinational drugs. Fufang zaofan pill was orally administrated twice daily at the dosage of 3 - 7 pills for at least 6 months; Androgen was orally given equally three times per day at the dosage of 0.1~0.2 mg/( kg·d) for at least 12 months; intravenous immumoglobulin was injected once a month for 6 times at the dosage of 1g/kg; cyclosporine was oraUy administrated averagely twice a day at the dosage of 5 - 8 mg/(kg.d) for at least 6 months. Results: Compared with prior treatment, WBC, RBC, HB and Rct resuited in blood routine test were increased 3 months post therapy, however, PLT was not significantly increased. After 6 and 12 months post treatmont, all indices in blood routine test were increased significantly when compared with those before the treatment and 3 months post therapy, although PLT was elevated slowly. 12 months post treatment, 10 (28.6%) children were fundamentally cured, 12 (34.3%) recovered, and 9 (25.7 % ) significantly improved, and 5 ( 11.4 % ) remained ineffective. The overall effective rate was 88.6 %. No sever side effects were found in all children. Conclusions: For childhood chronic aplastic anemia, the combinational therapy of Chinese herb of fu fang zao fan pill and western o drugs of androgen, intravenous immunoglobulin and cyclospmine is effective and worthy of wider adoption.
