摘要
目的探讨不同孕期妊娠对肌酐(Cr)、尿素氮(BUN)、肾小球滤过率(GFR)的影响。方法对246例早孕者、520例孕足月者及146例非妊娠者测定肾功能、血浆蛋白、身高、体重。通过MDRD7方程计算GFR。分析早孕组、孕足月组与对照组Cr、BUN及GFR差异。结果早孕组Cr明显低于孕足月组及对照组[(56±12)μmol/L VS(62±12)μmol/L、(64±12)μmol/L]。早孕组GFR显著高于孕足月组及对照组[(112±30)m l.m in-1.(1.73m2)-1VS(92±18)m l.m in-1.(1.73m2)-1、(90±20)m l.m in-1.(1.73m2)-1]。结论妊娠早期Cr、BUN等明显下降,肾小球滤过显著增加。孕足月时Cr、GFR恢复至非妊娠状态。GFR较Cr判断肾功能更敏感、更准确。
Objective To investigate the effect of different pregnancy period to creatinine(Cr) ,urea nitrogen(BUN) ,glomerular fihration rate(GFR), Methods Two hundred and forty - six early pregnancy women,520 term pregnancy women, 146 non - pregnancy women were detected renal function, plasma protein, body height, weight. By MORD7 equation to calculate GFR, analysed the difference among the early pregnancy group, term pregnancy group,and control group about Cr,BUN ,GFR. Results The level of Cr in the earl) pregnancy group was lower than that in the term pregnancy group and control group[ (56 ± 12) μmol/L VS (62 ± 12)μmol/L, (64±12)μmol/L]. The level of GFR in the early pregnancy group was higher that in the term pregnancy group and control group[(112±30)ml · min^-1· (1.73m^2)^-1 VS (92±18)ml · min^-1 · (1.73m^2)^-1,(90±20)ml ·min^-1· ( 1.73m^2 ) ^- 1 ]. Conclusion In the early pregnancy, the level of Cr, BUN is significantly decreased, then the level of GFR is significantly increased. The level of Cr,GFR in the term pregnancy return to the level of the non - pregnancy state. GFR can judge renal function more sensitively and exactly than Cr.
