摘要 目的 评价眶上匙孔入路切除小型鞍结节脑膜瘤的技术和效果。方法分析我科2003年1月~2005年12月,经眶上匙孔入路切除鞍结节脑膜瘤15例,有限的眶上开颅宽25~30mm,高15~20mm,讨论肿瘤的大小、手术技巧、手术结果和并发症。结景尽管开颅小,但可提供足够的空间进行颅内操作,保护脑和其他重要结构。该组患者出院时恢复良好,肿瘤全切除率达93.3%,术后视力改善者达86.7%,没有与人路相关的术后并发症。结论对于有经验的神经外科医生来说,眶上匙孔人路是用于切除最大径〈3cm鞍结节脑膜瘤安全有效的方法。 Objective To evaluate the technique and effect of supraorbital keyhole approach for removing tuberculum sellae meningiomas. Methods An analysis of 15 patients with tuberculum sellae meningiomas removed by supraorbital keyhole approach from January 2003 to December 2005 was made. The technique was described in detail, a limited supraorbital craniotomy was performed with a width of 25 to 30 mm and a height of 15 to 20 mm. Tumour size, operative techniques, operative result and complications were discussed. Results Despite the small size of the craniotomy, the exploration allowed enough room for intracranial manipulation with maximal protection of brain and other intracranial structure. The presented series of patients gained good recovery with 93.3% of total remove rate and 86.7% of improved visual without any approach-related complications post-operatively. Conclusion Supraorbital keyhole approach is safe and effective for an experienced neurosurgeon to remove tuberculum sellae meningiomas which is less than 3 cm in diameter.
