摘要
目的探讨等离子刀治疗间盘源性颈肩痛的手术技巧和提高疗效的措施。方法2002年12月至2005年5月,34例间盘源性颈肩痛患者(68个椎间盘)接受了等离子刀汽化消融髓核成形术治疗,其中,男19例,女15例,平均年龄44岁。采用VAS评分及患者主观满意度评估疗效。结果全部病例均获随访2月~3年,术前VAS评分为6.64±0.53,随访时为2.47±0.69,与手术前比较差异有统计学意义(P<0.01),主观满意度优良率为94%,发生椎间盘炎1例。结论等离子刀汽化消融髓核成形术是治疗间盘源性颈肩痛的有效措施,此法创伤小、安全、疗效佳。提高操作技术、严格掌握手术适应证、详细的影像学数据是降低并发症发生率、提高疗效的关键。
Objective To evaluate the effects of iso-ion vaporization nucleoplasty on the cervical disc herniation and to investigate the methods for decreasing complications and improving efficacy. Methods A total of 34 patients with cervical disc herniation were treated with iso-ion vaporization nucleoplasty from December 2002 to May 2005, their effects were evaluated using the VAS and the subjective satisfactory degree of the patients. Results All the patients were followed up from 2 months to three years. Averaged preoperative VAS was 6. 64 ± 0. 53, postoperative 2. 47 ± 0. 69, the pain relief was statistically significant (P〈0. 01 ). The subjective satisfactory degree of the patients was 94%. There was one case of disc infection. Conclusion Iso-ion vaporization nucleoplasty is an effective, minimally invasive and safe procedure to treat cervical disc herniation. Factors directly related to effectiveness include: improving operative techniques, strictly following indications and adequate imaging data.
