后路内窥镜结合可扩张通道管手术系统治疗椎间盘突出伴腰椎不稳疗效分析 被引量:3

摘要 目的探讨运用后路内窥镜结合可扩张通道管手术系统(X—Tube)对退行性椎间盘突出伴腰椎不稳的治疗效果。方法对8例退行性椎间盘突出伴腰椎不稳患者运用后路内窥镜结合X—Tube行经单侧椎间孔入路椎管减压、椎间植骨融合、椎弓根螺钉复位内固定术(TLIF)治疗,另一侧仅行椎弓根螺钉复位内固定术治疗。结果手术时间3—5小时。术中出血180—400ml,所有手术切口均1期愈合,无神经功能损伤等并发症。术后随访8~18月,患者腰腿疼痛较术前明显缓解或消失。结论该方法切口小,术中软组织剥离轻,术中出血和术后引流少,术后伤口疼痛轻;但手术时间长,X射线量大,操作难度较高,应严格掌握适应讧。 Objective To discuss the efficacy of METRX X-Tube in the treatment of protrusion of retrograde intervertebral disk combined with unstable lumbar vertebra. Methods Eight patients were treated by decompression of spinal canal from intervertebral foramen, interbody fusion and transforaminal lumbar interbody fusion (TLIFI on one side and only TLIF on the other side by use of METRX X-Tube. Results The duration of operation was 3 to 5 hours and the hemorrhage volume during operation was 180 to 400 ml. All the incisions were achieved primary healing and there were no complications such as impairment of nerve function. In the follow-up of 8 to 18 months after operation, lumbocrural pain has obviously alleviated. Conclusion The advantages of METRX X-Tube include small incision, mild stripping to the soft tissue, less hemorrhage during operation and less drainage after operation, minimal wound pain. Its drawbacks consist of long operation duration, large amount of X ray exposure and difficulty to manipulate. We espouse a strictly elective indication.
