摘要
目的评价骨科住院病人管理流程改善的效果。方法从2004年8月-2005年8月在科室成立“骨科住院病人管理流程改善”小组,分析科室现有的管理流程缺陷,确定造成缺陷的主要原因并制定相应的措施:改变传统的医生固定管理床位;规范入院流程;加强安排病人检查计划性和组织实施。结果住院病人管理流程改善使病人在院外等候入院的时间从改善前的28.5 d降低到4.5 d,降低了24 d,下降率为84.2%;病人的平均住院时间从改善前的21.5 d降低到11.4 d,降低了10.1 d,下降率为46.9%;病人的住院费用从5 346.00元降低到3 428.00元,降低了1 918.00元,下降率为35.8%;病人的满意度从改善前80.5%上升到95.8%,上升了15.3%。结论对骨科住院病人的管理流程进行改善,降低病人院外等候入院时间及住院费用,减少病人的平均住院日,提高病人的满意度。流程改善活动在科室住院病人管理中应用取得较好的效果,值得推广。
Objective To assess the effect of improving the flow-sheet for managing patients in the department of orthopaedics. Methods A group in charge of ＂improving the flow-sheet for managing patients in the department of orthopaedics＂ was set up in Aug. 2004 to Aug. 2005, specialized in analyzing the flaws in the current managing flow-sheet, identifying the lead causes and regulating corresponding measures, changing the bed management by designated doctors, standardizing check-in flow-sheet and arranging for and conducting intensified plans for checking patients. Results The time for patients to wait for admission into the department was shortened from 28.5 d to 4.5 d, with a falling rate of 84.2% and the average duration of hospitalization fell from 21.5 d to 11.4 d, with a failing rate of 46.9%. The expense for hospitalization fell from 5 346.00 yuan to 1 918.00 yuan, with a falling rate of 35.8%. The comfort level was increased from 80.5% to 95.8%, with an increasing rate of 15.3%. Conclusion The improved flow-sheet can lower the hospitalization fee, reduce the average duration in hospital and meanwhile, raise the comfort degree from patients. The application of the improved flow-sheet is effective and worth popularizing.
