孕妇早产与病原生物感染的相关性

Study on the relationship between pathogen infection and the premature labor in pregnant women

摘要 目的分析梅毒螺旋体(TP)、解脲支原体(UU)、沙眼衣原体(CT)及弓形体(Tox)、风疹病毒(RuV)、巨细胞病毒(CMV)、单纯疱疹病毒(HSV)(合称TORCH)系列病原生物感染与早产的相关性。方法对315例早产妇女和100例正常育龄而无自然流产史妇女,采用酶联免疫法(ELISA)进行梅毒螺旋体和TORCH系列病原体血清IgM抗体检测,同时采用胶体金快速检测法和培养法进行宫颈分泌物CT抗原检测和UU分离。结果早产组血清TP、Tox、CMV、RuV、HSVIgM阳性率分别是8·57%、16·19%、23.65%、20.00%、39·05%、对照组阳性率分别是0、2.00%、7·00%、4.00%、9.00%;宫颈分泌物UU、CT抗原检出率早产组分别为37.78%、13·65%,对照组分别为18%、5%,对照组与早产组差异显著(P<0·05)。结论TP、UU、CT及TORCH系列病原微生物感染是引起孕妇流产的一个不可忽视的重要原因,孕前及孕期进行相关病原微生物检测,对保证胎儿健康出生具有重要的临床意义。 Objective To study the relationship between a series of pathogenic microorganism infection in pregnant women and premature outcome. Methods The sera from 315 pregnant women of premature birth and 100 normal pregnant women were tested for detection of antibodies to Treponema palidum (TP), Ureaplasma urealyticum (UU), Chlamydia trachomatis ( CT), Toxoplasma gondii (Tox), Rubella virus(RuV), cytomegalovirus(CT) and Herpes simplex virus(HSV) with an indirect enzyme linked immunoassay, and their cervical secretions were detected for CT and UU antigen by colloidal gold assay. Results The positive rates of specific IgM to TP,Tox, CMV, RuV and HSV in serum samples were 8.57%, 16.19% ,33.65% ,20% and 39.05% respectively; while that in the control group were only 0,2% ,7% ,4% ,9% . The pathogen antigen detection rate of UU and CT in the cervical secretions in premature birth pregnant women 37.78% ,CT 13.65% respectively and that in 100 normal pregnant women were 18% and 5 % respectively, showing significant difference ( P 〈 0. 005). Conclusion These pathogen infections are associated with increased risk of premature birth. Thus detection of relevant pathogens is recommended in pregestational and gestational periods for ensuring the health of the fetus and the mature birth of the baby.
