摘要 目的:探讨在CT引导下经小关节内侧缘穿刺,注入臭氧及胶原蛋白酶联合应用治疗腰椎间盘突出症的临床研究.方法:45例经临床诊断为腰椎间盘突出症患者,在CT引导下穿刺经小关节内侧缘进入椎间盘内,注入浓度为60μg/ml的臭氧10ml,退针至侧隐窝注入40μg/ml的臭氧5ml和胶原蛋白酶600~1200u.结果:观察患者术后臭氧分布及1个月~6个月的治疗效果,疗效根据改良的Macnab方法评价,其疗效优、良、差率分别为40%、53%、7%.结论:CT引导下操作简单,准确安全,腰椎间盘内注入臭氧及侧隐窝注入臭氧和胶原蛋白酶联合应用是一种微创介入治疗腰椎间盘突出症的一种有效方法. Objective:To discuss the clinical research of CT-guided combined with injection of ozone and collagenasc in the treatment of lumbar intervertebral disc herniation through inner margin of small joint. Methods: All 45 patients with lumbar intervertebral disc herniation were injected 10ml of 60μg/ml ozone under the guidence of CT. The needle punctures through the inner margin of diapophysis and gets in intervertebral disc. Then the needle is pulled back at lateral crypt fossa; 5ml of 40μg/ml ozone and 600 - 1200u collagenese are injected.Results: Observing the location of ozone after operation and the therapy effect during 1 -6 month. Evaluating therapy effect according to Macnab criteria, and the best, good, and poor therapy rate is 40%, 53% and 7% respectively. Conclusion:Operation under CT-guidence is easy, safe and accurate. The therapy of injection of ozone in intervertebral disc and combined injection of ozone and collagenase in lateral crypt fossa is a minimally invasive and effective method.
