摘要
为探讨预防性关节内注射丹参及丹参加玻璃酸钠对关节粘连的影响,将成年健康日本大耳白兔37只随机分为4组:A组9只,B组9只,C组10只,D组9只。手术切开右后膝关节,搜刮髌上囊前壁,推开髌上囊后壁在股骨上的附着,切除髌下脂肪垫,缝合关节囊及皮肤后关节穿刺,A组每只动物注生理盐水0.3ml、B组注玻璃酸钠0.3ml、C组注丹参注射液0.3ml、D组注丹参0.15ml加玻璃酸钠0.15ml,同时用管形石膏固定患肢于屈膝70—90°位;4周后处死动物,行关节活动度测量、关节腔行大体观察和髌上囊前壁、髌韧带后粘连组织的组织学观察和细胞凋亡检测。结果显示,关节活动度B、C组大于A组(P〈0.01),D组大于B、C组(P〈0.05);关节粘连计分B、C、D组均于小A组(P〈0,01);髌上囊前壁滑膜病变组织学计分B、C、D组均小于A组(P〈0.01);髌韧带后粘连组织成纤维细胞计数B、C、D组均小于A组(P〈0.01);髌上囊前壁滑膜凋亡细胞计数B、C组大于A组(P〈0.01),D组大于B、C组(P〈0.05);髌韧带后粘连组织凋亡细胞计数D组大于B、C组(P〈0.01),B、C组大于A组(P〈0.01)。说明丹参能有效顸防关节粘连,与玻璃酸钠的效果类似;丹参能加强玻璃酸钠预防关节粘连的效果。
The objective of the paper is to study the influence of preventive intra-articular injection of Radix Salviae Mitorrhizae(RSM) and Sodium Hyaluronate(SH) on joint adhesion.Thirty-seven Japanese healthy adult white rabbits were made into animal models of knee adhesion and randomly divided into four groups: Group A(n= 9) treated by 0.3-ml normal saline injection,Group B(n= 9) treated by 0.3-ml SH injection,Group C(n= 10) treated by 0.3-ml RSM injection and Group D(n= 9) treated by 0.15-ml RSM and 0.15-ml SH injections;four weeks later,measurement of the withered cell and joint activity,gross observation of the joint cavity and histological observation of the adhesive tissue were carried out.The results showed: the joint activities of Group B and Group C > the one of Group A(P < 0.01) and the one of Group D > the ones of Group B and Group C(P < 0.05);the joint-adhesion and synovial-lesion-histological scores and adhesive-tissue-fibroblasts counts of Group B,Group C and Group D < the ones of Group A(P < 0.01);the synovial-withered-cells counts of Group B and Group C > the one of Group A(P < 0.01) and the withered-cells counts of Group D > the ones of Group B and Group C(P < 0.05) and so were the witheredcells counts in the adhensive tissues(P < 0.01),suggesting that RSM has the effect similar to that of SH and could effectively prevent joint adhesion and strengthen the preventive effect of SH.
