摘要
目的观察急性脑梗死(ACI)早期辛伐他汀治疗对血脂和炎症反应标志物C!反应蛋白(CRP)的影响及差异,探讨他汀类药物在急性脑梗死早期治疗中的作用。方法选择急性脑梗死住院患者96例,随机分为辛伐他汀治疗组和非辛伐他汀治疗组,比较入院前和出院后甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL!ch)、高密度脂蛋白胆固醇(HDL!ch)和CRP水平。同期选取15例健康者作为对照组。结果①出院时辛伐他汀组与非辛伐他汀组的血浆CRP为(9.32±6.40)mg/L和(12.22±7.18)mg/L,显著低于入院时的(28.12±25.62)mg/L和(27.98±27.01)mg/L(P<0.01);出院时辛伐他汀组血浆CRP亦显著低于非辛伐他汀组(P<0.01)。②据入院时血浆CRP水平,将急性脑梗死患者分为CRP高值组和低值组,出院与入院时比较,高值组和低值组血浆CRP均显著降低(P<0.01),但只有高值组出院时辛伐他汀治疗者血浆CRP显著低于非辛伐他汀治疗者(P<0.05)。结论急性脑梗死患者早期辛伐他汀治疗使CRP显著降低,有明显抗炎症作用。
Objective To observe the effect and difference to C-reactive protein (CAP), for indicator of inflammation, and blood lipid level in acute cerebral infarction (ACI) patients treated with Simvastatin and discuss the impact of early treatment of ACI using statin. Methods 96 ACI in-patients were randomly divided into the two groups: one group was treated with Simvastatin and the other was not treated with Simvastatin. Their blood levels of TG, TC, LDL-ch, HDL-ch and CRP were compared before hospitalization and on discharge. 15 healthy people were selected as the control group. Results ① Comparing before admission and on discharge, the blood CRP level were reduced markedly in both'group, in Simvastatin treatment group and non-Simvastatin treatment group, the blood CRP of the former reduced significantly compared to that of the latter on discharge (P〈0.01). ② Based on the CRP level ACI patients could be divided to the high level and low level. The blood CRP level was reduced markedly (P〈0.01) in both group comparing that before admission and on discharge. However, only the blood CRP level of the high-CRP-level group in the Simvastatin treatment group reduced markedly on discharge comparing to the non-Simvastatin group (P〈0.05). Conclusions The blood CRP level reducess markedly in ACI patients treated with Simvastasin, this suggests that Simvastatin has strong anti-inflammatory effect.
