医源性桡动脉早期假性动脉瘤诊治

摘要 目的评价彩色多普勒超声和桡动脉加压板对早期医源性桡动脉假性动脉瘤的诊断和治疗价值。方法对经桡动脉入路行冠状动脉介入治疗术后约2~8小时内发生的前臂局部肿胀、疼痛部位,以彩色多普勒超声诊断假性动脉瘤是否存在;及其数量、大小、漏口部位等,于体表标记瘤的部位。用桡动脉加压板在标记处逐渐加压,然后加压固定之。经6小时后在彩色多普勒下复查。若动脉瘤未完全闭合,随时调整位置和压力至达闭合止。结果用6~26小时不等时间,经一次或几次的加压板加压固定压迫,3例均达瘤体闭合,漏口消失。均各在一月后复查,未见复发。结论彩色多普勒超声对医源性早期桡动脉假性动脉瘤确诊是安全、方便、可靠的。以桡动脉加压板加压固定治疗假性动脉瘤不仅可靠而且方便,同时不影响术后活动,不失为好的治疗方法。 Objective Evaluating the efficiency of diagnosis and curing PPORA with Color Doppler Ultrasound (CDUS) and Radial Artery Compression Board (RACB). Methods The patients who were found to have PPORA interventional procedure were treated with CDUS on the swelling forearms about 2-8 hours, then were checked and marked before being compressed with CDUS before being fixed, and were re-checked with CDUS after 6 hours (the treatment includes: diagnosis PPORA existing, its amount, size, Location and so on). If PPORA is not cured completely, then adjust with RACB for several times until PPORA is repaired. Result 3 cases with PPORA were repaired successfully after 6-26 hours treatment, all of them had no complication and recurrence in one month. Conclusion This treatment is a non - invasive, efficient and safe therapy for PPORA. For most PPORA cases, this treatment should be the first fine of therapy.
