经皮自体骨髓移植治疗儿童骨不连 被引量:6

Percutaneous Autogeous Bone Marrow Grafting For Treatment of The Ununion of Fractures of Children

摘要 目的探讨应用自体骨髓经皮注射治疗儿童骨不连的临床效果。方法对21例因先天性假关节及创伤骨折导致骨不连的患儿,于髂前上棘处用骨髓穿刺针抽吸骨髓10 ̄15ml,将之注射至骨不连的部位。术后常规用石膏或外固定架固定,定期X线摄片随访。结果16例在注射后3 ̄9个月内临床愈合,1例胫骨创伤骨折骨不连经多次注射在2年后愈合,4例未愈,1例病人失随访。结论自体骨髓移植治疗儿童骨不连有效,创伤小,操作简便,医疗费用低,可供临床选择。 Objective To disscuss and evaluate the method and clinic effect of percutaneous autogeous bone marrow injection on 21 cases of ununion of fracture. Methods Twenty-one ununion cases from congenital pseudarthrosis (n=7) and original trauma fracture (n=14) were enrolled. Age ranged from 1.5 to 13 years old.The patients were placed by prostration and the 10-15ml bone marrow was aspirated from the anterior superior lilac spine.h was than injected into the ununion region under C-arm X-ray fluoroscopy localization,using gypsum or external fixator to fix the affected limb after the operation. All the patients were regularly followed up by X-ray photograph. Results 16 cases were clinical healed in 3-9 months after bone marrow injection,one case of tibia trauma ununion ,who underwent multiple injections, at last also headed two years after the therapy. Failure cases in 4, one patient was not followed-up. Conclusion Autogeous bone marrow grafting is effective to treat ununion and has many advantages,such as easiness, less trauma and low medical cost.Therefore,it has popularizing value for treatment of fracture nonunion.
