保心汤对心肌梗死PTCA患者血清VEGF的影响 被引量:7

Influence of Baoxin Decoction on serum VEGF in patients with myocardial infarction after PTCA

摘要 目的研究中药配合介入治疗心肌梗死对患者血清血管内皮生长因子(VEGF)与心功能改善的影响。方法选择54位患者,随机分成3组:对照组8例,经皮冠状动脉内成形术(PTCA)组22例,保心汤组24例。用酶联吸附免疫法(ELISA)检测患者介入后0、1、7、14、21 d血清VEGF浓度,彩色多普勒超声心动仪测定介入后1、21 d左室射血分数(LVEF)。结果介入后血清VEGF浓度逐渐上升,1 d达到高峰,而后逐渐下降;PTCA组与保心汤组在0、1、7、14 d血清VEGF浓度无明显差别,但均高于对照组(P<0.01),第21天时保心汤组VEGF浓度高于PTCA组(P<0.01),并明显高于对照组,而PTCA组与对照组无明显差异;介入后1 d,PTCA组与保心汤组LVEF无明显差别,均低于对照组(P<0.01);第21天保心汤组高于PTCA组(P<0.05)。结论保心汤可能是通过调节VEGF表达,使其保持一个适合的血清浓度,以促进缺血区侧支循环的建立,保护受损心肌,促进心功能的恢复。 Objective To study the influence of Chinese herbal drugs on serum vascular endothelial growth factor (VEGF) and heart function in patients with myocardial infarction (MI). Methods To divide randomly 54 cases into 3 groups, including the control group with 8 cases ( groupl ) , percutaneous transluminal angioplasty (PTCA) group with 22 cases (group2) and Baoxin Decoction group with 24 cases (group3). The concentration of serum VEGF was detected by enzyme - linked immunosorbent assay (ELISA) on 0^th, 1^st, 7^th, 14^th and 21^st day respectively after the treatment. The colour Doppler ultrasonic cardiograph was used for detect the left ventficular ejection fraction (LVEF) on 1^st and 21^st day after the treatment. Result The concentration of serum VEGF increased gradually after the beginning of the treatment, reached the peak on 1^st day and then decreased gradually. The concentration of serum VEGF was not significant different between group2 and group3 on 0^th, 1^st, 7^th and 14^th day, but higher than that of groupl (P 〈 0.01 ). On 21^st day the concentration of serum VEGF in group3 was higher than that in group2 ( P 〈 0.01 ) and obviously higher than that in groupl, but there was no significant difference between group2 and groupl. On 1 st day LVEF was not significant different between group2 and group3 and lower than that in groupl (P 〈0.01 ). On 21^st, day LVEF was higher in group3 than that in group2 (P 〈0.05). Conclusion Baoxin Decoction can promote the collateralization in ischemic zone, protect cardiac muscle and improve the recovery of heart function through regulating the VEGF expression and keep VEGF a suitable serum concentration.
