摘要
目的寻找适合老年前列腺气化电切手术截石体位的上身摆放方法,分析影响患者血压变化的有关因素。方法选取符合纳入标准的手术患者120人,按4种不同上身体位进行随机分组摆放,记录手术结束后体位改变引起的血压变化值。结果术中患者上身体位的摆放对术后血压变化的影响基本上无差异;体重、手术时间长短、术中输入的液体量、有无高血压病史与术后体位改变后血压的变化有关,而年龄、术中膀胱冲洗液量、是否为改良的截石位与术后体位改变后血压的变化无关。结论老年前列腺气化电切手术截石体位术后的确会引起患者血压较大的波动,建议术中以10°的上身体位进行摆放。
Objective To explore the best upper body position for patients undergoing the transurethral vaporization of the prostate （TUVP）, and the factors influencing the patients＂ blood pressure. Methods One hundred and twenty cases were selected into the study according to the sample criteria. They were randomly divided into 4 groups with different upper body positions. The changes in blood pressure resulting from the changes in position were recorded. Results The patients＂ upper body position during operation had influence on the changes in blood pressure. But there was little difference among them. Factors influencing changes in post-operative blood pressure were body weight, operation duration, fluid input, and previous history of hypertension. Other factors, such as age, volume of bladder irritation during operation, and modified lithotomy position, had no relation with the changes in post - operative blood pressure. Conclusion It will indeed cause blood pressure change a lot when old patients take lithotomy position under TUVP. The appropriate position should be 10 degree of upper body position.
