摘要
目的 探讨术前介入化疗联合根治术治疗Ⅰb-Ⅱa期巨块型宫颈癌的近期疗效.方法 38例Ⅰb-Ⅱa期巨块型宫颈癌患者均以卡铂300 mg/m2、平阳霉素50 mg/m2、长春新碱1 mg/m2进行介入治疗,经1～2个疗程后行宫颈癌根治术.结果 38例介入总有效率为89.5%,其中完全缓解CR 7例,占18.4%;部分缓解PR 27例,占71.1%.无效SD 3例,肿瘤进展PD 1例.CR中无一例发生淋巴结转移,PR中6例发生术后淋巴结转移.38例均未发生严重的与化疗和手术相关的并发症.结论 术前介入化疗可缩小肿瘤病灶,减少淋巴结转移,有助于提高Ⅰb-Ⅱa期巨块型宫颈癌的生存率.
Objective To study the feasibility of intra - arterial chemotherapy before radical excision for patients with massive cervical cancer in stage Ⅰb - Ⅱa. Methods 38 cases with massive cervical cancer in stage Ⅰb - Ⅱa were treated with 1 ～ 2course of bilateral internal iliactery infusion of carboplatin 300mg/m^2 ,doxorubicin 50 mg/m^2 and vincristine 1 mg/m^2 before radical hysterectomy. Results The total response rate was 89,5%, complete response（CR） was noted in 7 cases（ 18.4% ） ;while partial response in 27 （71.1% ） . 3 cases had no change and 1 case had progressive disease. 6 patient with PR were found pelvic lymphnode metastasis. There were no severe complications related to intra - arterial chemotherapy and radical hysterectomy. Conclusion Intra - arterial chemotherapy before operation can shrink tumorsize,decrease the lymphnode metastasis. It is helpful in elevating the survival rate of patients with massive cervical cancer in stage Ⅰb - Ⅱa.
