摘要
目的探讨TPO蛋白在甲状腺乳头状癌与结节性甲状腺肿伴乳头状增生鉴别诊断中的作用。方法用免疫组化EnVision法测定50例甲状腺乳头状癌及50例结节性甲状腺肿伴乳头状增生的TPO表达情况。结果50例甲状腺乳头状癌中有48例TPO(-),2例(+);50例结节性甲状腺肿伴乳头状增生中TPO均(+)。TPO在甲状腺乳头状癌与良性乳头状增生鉴别诊断中的评价指标:敏感度为96%,特异度为100%,准确度为98%,阳性预测值为100%,阴性预测值为96.15%。结论TPO蛋白可作为鉴别甲状腺乳头状癌与良性乳头状增生的重要参考标记物。
Objective To study the role of thyroperoxidase （TPO） in the different diagnosis between malignant and benig nthyroid papillary hyperplasia. Methods The expression of TPO protien in 50 cases of papillary thyroid carcinomas and 50 eases of thyroid nodular goiter with papillary hyperplasia were dectected by inmmunohistochemistrieal EnVision Method. Results Of the 50 eases thyroid papillary carcinomas, 48 were negative and 2 were positive for TPO; while all of the 50 eases of nodular thyroid goiters with papillary hyperplasia were positive for TPO. The values of TPO in the different diagnosis between malignant and benign thyroid papillary hyperplasia were assessed below： sensitivity was 96%, specificity was 100%, accuracy was 98%, ＋ PV was 100%, and - PV was 96.15%. Conclusion TPO protein is a useful marker in the differential diagnosis between malignant and benign thyroid papillary hyperplasia.
