摘要
目的:了解静息心电图正常胸痛患者的冠脉病变特征。方法:分析58例静息心电图正常胸痛患者的临床资料与冠脉病变情况,并与同期294例静息心电图异常胸痛患者相比较。结果:与静息心电图异常胸痛患者相比,静息心电图正常胸痛患者以单支病变多见[67.2%(26/39)对40.0%(98/294),P=0.001],多血管病变较少[32.8%(13/39)对60.0%(144/294),P=0.001],在13例多血管病变中,有11例(84.6%)血管狭窄部位相互对应,平均冠脉病变支数较少[(1.89±0.57)对(1.97±0.63),P=0.004],Gensin i积分低[(34.2±24.65)对(56.2±26.78),P<0.001];在39例单支病变中,以前降支病变多见[61.5%(16/26)];在冠脉造影阳性率两组基本相同,但性别比较以男性多见,(65.1%(183/281)对21%(15/71),P=0.000),三个以上危险因素方面多见,(81.5%(229/281)对31%(22/71),P=0.000)。结论:静息心电图正常胸痛患者以单支血管病变多见,冠脉病变不太严重;在多血管病变中,大部分血管狭窄部位相互对应;在胸痛患者中男性、危险因素聚集趋势是冠脉病变的危险因素之一。
Objective :To investigate the characteristics of coronary angiography of patients suffering from chest pain with normal ECG. Methods: Fifty - eight patients with chest pain and normal ECG and two hundred ninety - four patients with abnormal ECG were studied by case -control method, whose clinical data and coronary angiography were analyzed. Results: Compared with the patients with chest pain and abnormal ECG, more single vessel diseases [ 67.2 % (26/39) vs 40.0% (98/294), P = 0.001 ] and less mutiple - vessel diseases[ 32.8% ( 13/39 ) vs 60.0% ( 144/294 ), P = 0.001, whose Gensini scoring was significantly lowered [ ( 34.2 ± 24.65) vs (56.2 ± 26.78) ,P 〈 0.001 ]. For those patients with single -vessel diseases, lesions are predominantly found in left anterior descending coronary artery [ 61. 5 % (16/26) ]. For those patients with mutiple - vessel diseases, (84.6%) ( 11/13 )had lesions at the opposite side of each vessel. The positive rate of CAG was not different in two groups, But in gender aspect there were different. The positive rate of CAG were predominantly found in male(65.1% ( 183/281 )vs 21% ( 15/71 ) ,p =0. 000). In vascular risk factor aspect the positive rate of CA G were predominantly found in 3 or above 3 risk factors (81.5% (229/281) vs 31% (22/71), P = 0. 000). Conclusions: For the patients with chest pain and normal resting ECG, the majority have single - vessel diseases, and their coronary lesions are less severe. For the patients with mutiple -vessel diseases,more lesions are found at the opposite side of each vessel. In chest pain patients male and focus of risk factors were predictors of prognosis of the severeness in coronary artery lesions.
