胰岛素泵治疗糖尿病酮症酸中毒的临床观察 被引量:15

摘要 目的比较胰岛素泵持续皮下胰岛素输注(CSII)法与传统小剂量胰岛素静滴法治疗糖尿病酮症酸中毒(DKA)的短期疗效。方法 DKA患者66例,随机分为CSII组和对照组,CSII组给予胰岛素泵持续皮下输注诺和灵R,对照组用小剂量诺和灵R持续静滴。结果 CSII组酮体转阴快(P<0.05),两组达到相同的血糖水平,CSII组所需的治疗时间、胰岛素用量以及低血糖发生率明显减少(P<0.05)。结论胰岛素泵治疗DKA酮体转阴快,能更快、更有效地控制高血糖,减少低血糖发生。 Objective To compare the short-term effects of continuous subcutaneous insulin infusion (CSⅡ) and continuous venous insulin infusion in the treatment of diabetic ketoacidosis (DKA). Methods 66 DKA patients were randomly divided into CSⅡ group and control group. In CSⅡ group, the Novolin R was subcutaneously infused using insulin pump continuously. The continuous venous infusion of Novolin R was administrated in control group. Results The time for ketonuria to clear in the CCSⅡ group was shorter than that in the control group ( P 〈 0.05). Therapeutic time, insulin dosage and insulin induced hypoglycemia reaction incidence in the CSⅡgroup were obviously decreased than those in control group ( P 〈 0.05 ). Conc, Conclusion Treatment with insulin pump infusion in DKA resulted in the faster disappearance of ketonuria. This could control hyperglycemia more effectively, more promptly.
