摘要
目的了解正常儿童股骨骨髓的MRI信号转化特点,为诊断骨髓病变提供帮助.方法将34例14个月~14岁儿童分三组,A组(14个月~4岁)11例,B组(5~9岁)11例,C组(10~14岁)12例.对股骨行冠状位T1WI及STIR序列扫描,测量近侧干骺端(PM)、骨干(D)、远侧干骺端(DM)及大腿肌肉(M)的信号强度,并计算PM/M,D/M及DM/M值.结果 T1WI A组的PM/M明显低于D/M及DM/M,D/M与DM/M无显著差异;B、C组的PM/M与DM/M均低于同组D/M,而PM/M与DM/M间无显著差异;B、C组的各部位信号均显著高于A组,而B、C两组间无显著差异.STIR序列中A、B两组内各部位间骨髓信号无显著差异,只有C组的PM/M明显高于D/M及DM/M;A组的D/M明显高于B组及C组,而PM/M及DM/M在三组间均无显著差异.结论 14个月~4岁儿童的近侧干骺端脂肪含量明显少于远侧干骺端及骨干;5~14岁儿童近侧干骺端和远侧干骺端的脂肪含量仍少于骨干,9岁后股骨各部位脂肪化速度减慢;14个月~14岁儿童股骨各部位骨髓脂肪化顺序由先到后依次为骨干、远侧干骺端及近侧干骺端;T1WI较STIR序列能更敏感地显示正常儿童股骨骨髓信号的变化.
Objective To describe the MR characters and conversion of the bone marrow signal of children's femur, and to serve as a diagnostic aid for accurate interpretation of the pathologic processes of children's bone marrow. Methods Thirtyfour children (14 m-14 y) was divided into three groups, group A (14 m-4 y) 11 cases, group B (5-9 y) 11 cases, group C (10-14 y) 12 cases, MR examinations of femur were performed. All children had Tl-weighted and STIR sequences coronal planes. The signal intensity of proximal metaphysis (PM), diaphysis (D), distal metaphysis (DM) and muscle (M) were quantified and the values of PM/M, D/M and DM/M were calculated. Results On TIWI, the values of PM/M of group A was significantly lower than D/M and DM/M, and there was no significant difference between D/M and DM/M; In both group B and C, PM/M and DM/M were much lower than D/M, and there was no obvious difference between PM/M and D/MM; PM/M, D/M and DM/M in group B and C were significantly higher than those in group A, but no statistical difference between group B and C. On STIR sequence, the values of PM/M was much higher than D/M and DM/M, there was no obvious difference among three values in both group A and group B; D/M of group A was significantly higher than those of group B and group C, whereas both PM/M and DM/M had no difference among three groups. Conclusion At 14 m -4 y old, the fatty marrow of proximal metaphysis is significantly less than those of distal metaphysis and diaphysis; At 5 -14 y old, the fatty marrow of proximal metaphysis and distal metaphysis is still less than that of diaphysis, and the conversion from hematopoietic to fatty marrow slowed down after 9-year-old; At 14 m-14 y old, the sequence of conversion occurred from diaphysis, distal metaphysis to proximal metaphysis; Tl-weighted images are more sensitive than those of STIR sequence in showing the signal differences in normal children's bone marrow of femur.
