摘要
目的:探讨严重眼外伤一期清创缝合联合前部玻璃体切除+前部成型术的手术疗效。方法:眼前节外伤患者17例行一期角巩膜清创缝合联合前部玻璃体切除处理。结果:本组7例一期处理,后节稳定,二期行人工晶状体植入,视力改善。9例经二期玻璃体切除+视网膜复位术处理后,视网膜复位。1例因经济原因放弃治疗,眼球萎缩。17例均保留了眼球,甚至不同程度提高视力。结论:严格掌握手术适应证,一期前部玻璃体切除可以预防并发症,为二期手术创造条件,从而提高脱残率。
AIM： To investigate the effectiveness of first-stage anterior vitrectomy in the treabnent of severe ocular trauma.
METHODS： The wound of 17 cases with severe ocular trauma were closed and the patients received anterior v＇rtrectomy in the same time.
RESULTS： Seven cases had secondary IOL implantation after the first-stage anterior vitrectomy and their visual acuity was improved. 9 cases had second-stage vitrectomy combined with retina detachment surgery and the retina was attached. The eyeballs of all cases were reserved, and the visual acuity was improved in different degree.
CONCLUSION： The first-stage vitrectomy can prevent complications caused by the trauma and make it easier to perfom the second-stage surgery, which would improve the prognosis.
