摘要
目的以流动社区康复中心为依托,应用宫颈刮片、阴道镜HPV等技术,对本社区常住已婚妇女进行宫颈病变筛查,以确认流动社区康复中心在宫颈病变早防早诊早治的地位和作用.方法2005年3月至5月,以流动社区康复中心为依托,以建立的户籍健康档案的电子网络管理为基础,固定专业技术人员,成立妇科普查小组,在流动社区康复中心辐射半径范围内到达居委、居民小组、渔船上、家中进行普查.结果普查1 273例人群中,急慢性宫颈炎487例（38.3%）,不典型鳞状细胞82例（6.7%）,HPV感染53例（4.16%）,不典型腺细胞16例（1.3%）,低度鳞状上皮内瘤变20例（1.7%）,高度鳞状上皮内瘤变12例（0.94%）,原位癌1例（0.08%）,浸润癌1例（0.08%）.结论流动社区康复中心车载设备,固定普查专业人员,深入居委、居民小组、渔船上、家中的普查模式是基层医疗机构进行妇科普查的非常完整的、灵活机动的可行方法,对深圳沿海边远山区常住已婚妇女的宫颈病变早发现、早诊断、早治疗中起着非常重要的作用,能及时发现急慢性宫颈炎、HPV感染,CIN发病率高,宫颈癌的发病趋于年轻化等宝贵的第一手资料.50～84岁之间妇女在这次普查中发现上皮内瘤变（CIN）、HPV感染率明显高于一般人群.
Objective To search an operable method of gynecopathy general investigation for basal medical organization,and to affirm the station and effect of moveable community healing center（MCHC） in prevention,diagnose and treatment of cervix pathological changes, based on the moveable community healing center, used the technical of PAP smear, colposcopy, HPV, to process the general investigation of cervix pathological changes among the married women. Methods Rdied on the MCHC, based on the cyber-healthy archives of registered permanent residence,steadying the professional personnel, formed a gynecopathy general investigation group, within the ability of MCHC,general investigation was taken at these locations：communities, blocks, fishermen and citizen＇s houses. Resalts Among the 1273 cases of general investigation,there were：487 cases with acute and chronic cervicitis,account for 38.3 %, 82 cases with atypical squamous cells, account for 6.7 %, 53 cases with HPV infection, account for 4.16%, 16 cases with atypical adenoid cell,account for 1.3% ,20 cases with low-grade squamous intraepithelial neoplasia,account for 1.7%, 12 cases with high-grade squamous intraepithelial neoplasia, account for 0.94%, 1 case with carcinoma in situ,account for 0.08 %, 1 case with invasive tumor, account for 0.08 %. Conclusions The general investigation mode of using the vehicular equipment of moveable community healing center,steadying the professional personnel, going deep into communities, blocks, fishermen and citizen＇ s houses is a completely, flexible and operational method for the basal medical organization to take gynecopathy general investigation. It is a very important function of the prevention, diagnose and treatment cervix pathological changes for the married women who live in the coastal ulterior region,and this kind of general investigation mode can discover the original data betimes,about acute and chronic cervicitis, HPV infection, CIN, cervical cancer. Through this general investigation we find that the rate of CIN and HPV infection in the women from 50 to 84 years old is obviously higher than in the normal crowd.
