摘要
目的：介绍一种矫正钮扣状畸形的新方法。作者称之为“折叠还原法”。方法：将断裂后由纤维组织相连而延长的中央腱束，游离后折叠缝合；用细克氏针将近侧指间关节固定于充分伸展位，在远侧指间关节处于 ４５度时，将两侧腱束还原到正常解剖位置缝合。结果：本组共 １９例，按 Ｃａｒｏｌｉ［１］的评定标准，优１２例，良４例，可３例，总优良率为８４％（１６／１９）。结论：该术式能恢复伸肌装置的正常解剖关系，手术简便，是矫正创伤后钮扣状畸形的一种有效方法。在选择该术式时，伤指指关节被动活动必须正常。
A 'fold back' procedure was introduced to correct post-traumatic boutonniere deformity. The lengthened central slip of extensor tendon was mobilized and folded back to be sutured with the proximal interphalangeal joint fixed in full extension by Kirchner's wire; and the distal interphalangeal joint held flexed at approximately 45 degrees. The lateral slips were put back to their normal anatomic position. 19 patients received this procedure. Excellent outcomes were obtained in 12 cases(63%), good in 4(21%) and fair in 3(16%). This technique retains the anatomic structure of extensor apparatus, is simple and effective to correct boutonniere deformity of fingers with normal passive movement.
