摘要
对56例发病初心电图ST段抬高(第Ⅰ组)和25例无ST段抬高(第Ⅱ组)的急性心肌梗塞(AMI)患者的磷酸肌酸激酶同功酶(CKMB)峰值进行对比。结果:第Ⅰ、Ⅱ组患者CKMB峰值分别为207±93U/L和63±49U/L(P<0.01),CKMB峰值时间分别为18±6小时和11±4小时(P<0.01)。无ST段抬高者其CKMB峰值较低,但出现时间提前。这一现象与AMI早期冠状动脉自发性复流的观点一致。
his study compared the time from the onset of symptoms to the peak of the creatine kinasemyocardial band (CKMB) in 81 patients with acute myocardial infarction.The patients were divided into two groups:group 1(n=56) had diagnostic ST segment elevation(≥0 1mV)on the initial ECG,and group 2 (n=25) did not have diagnostic ST segment elevation on the initial ECG.In group 1,the time to the peak CKMB was 18±6 hours vs 11±4 hours in group 2( P <0 01).Peak CKMB was 207±93U/L in group 1 vs 63±49U/L in group 2 ( P <0 01).In group 1,49 patients subsequently had a Q wave myocardial infarction whereas only 7 patients in group 2 ( P <0 01).Patients without diagnostic ST segment elevation on the initial ECG had an early but low peak of CKMB with a typical nonQ wave infarction.These findings were consistent with early spontaneous restoration of blood flow during the infarction process in these patients.The fact of early restoration of blood flow may explain the high incidence of recurrent ischemic events noted in patients with nonQ wave myocardial infarction.
